老年癌症患者疼痛程度及其对生活质量的影响

目的探讨老年癌症疼痛(以下简称癌痛)患者疼痛程度对生活质量的影响,分析其影响因素,为提高老年癌痛病人生活质量(qualityoflife,QOL)提供依据。方法采用长海痛尺、CPQOL评分表对84例老年住院癌痛病人进行疼痛程度及生活质量评估,并分析癌症疼痛主要影响生活质量的哪些方面。结果老年癌痛患者中、重度疼痛发生率为50%,随着疼痛程度的加重,癌症患者生活质量下降,有统计学意义(F=45.567,P<0.0001),主要表现在睡眠、精神、疲劳、日常生活、食欲方面。经三阶梯止痛法积极治疗及健康宣传教育后,95%患者疼痛缓解,生活质量明显改善,治疗前后比较有显著性差异(P<0.0001)。结论应高度重视对老年癌症患者疼痛的评估,根据中、重度疼痛对患者躯体、精神、心理等方面的影响,尽早给予有效的药物治疗、心理护理、疾病宣教,努力减轻患者的疼痛,提高老年癌症患者的生存质量。

Study on pain of senile cancer patients and factors influencing life qualities

Objective To study influence of pain degree of elder cancer patients on their quality of life (QOL) and to analyze its influencing factors,so as to provide data for improving the QOL of the elder patients. Methods Pain degree and QOL of 84 hospitalizing elder cancer patients were assessed by Changhai Pain Measuring Scale and CPQOL and influences of cancer pain on patients' QOL were analyzed. Results Severe pain occurred in 50% elder patients and patients' QOL mainly including sleeping,mood,fatigue,daily life and appetite decreased with the increase of pain,which showed statistical significance (F=45.567,P<0.0001). After 3 phases positive treatment of killing pain and health education,pain of 95% patients was alleviated and quality of life was improve. There was significant difference between before and after treatment (P<0.0001). Conclusion It is necessary to pay great attention to pain assessment of elder patients. According to influence of medium and severe pain on patients' body,mentality and psychology,effective medical treatment,psychological nursing,health education about disease should be given as early as possible as so to alleviate patient's pain and improve their QOL.
