直肠癌组织中CXCR4和VEGF-C表达的临床意义

目的探讨趋化因子受体CXCR4（CXC chemokine receptor 4,CXCR4）和血管内皮生长因子-C（vascu-lar endothelial growth factor C,VEGF-C）在人直肠癌组织、癌旁组织和淋巴结中的表达及意义。方法采用免疫组织化学方法检测10例癌旁正常黏膜组织、50例癌组织和相关淋巴结中CXCR4与VEGF-C的蛋白表达情况,并分析其与临床病理特征之间的关系。结果 CXCR4与VEGF-C在直肠癌组织中均有高表达,CXCR4的表达率为72%,VEGF-C的表达率为92%,与癌旁正常组织相比差异均有统计学意义。转移淋巴结中CXCR4表达的阳性率为95%,VEGF-C表达的阳性率为93%,与无淋巴结转移组相比差异均有统计学意义。同时CXCR4和VEGF-C的表达与癌组织的不同侵润程度、组织分化程度和病理分期之间有密切关系,且两者呈正相关。结论 CXCR4和VEGF-C在直肠癌组织和转移淋巴结中的表达呈正相关,提示CXCR4和VEGF-C在直肠癌肿瘤的侵润、转移过程中起协同作用,并可作为评估直肠癌组织淋巴结转移的指标之一。

The clinical significance of the expression of CXCR4 and VEGF-C in human rectal tumor tissues

Objective To investigate the expression of CXC chemokine receptor4（CXCR4） and vascular endothelial growth factor C（VEGF-C） in human rectal tumor tissues,cancer adjacent tissues,lymph node and to explore the clinical significance.Methods The expression of CXCR4 and VEGF-C was determined by immunohistochemistry in adjacent normal mucosa of 10 cases and in rectal cancer tissues and relevant lymph nodes of 50 cases.The correlation of the 2 kinds of protein and their relationship with clinicopathologic characteristics were analyzed.Results CXCR4 and VEGF-C were highly expressed in rectal tumor tissues.The positive expression of CXCR4 in rectal cancer was 72%（P〈0.05）,and that of VEGF-C was 92%（P〈0.01）.There was significant statistical difference compared with the normal cancer adjacent tissues.The positive expression of CXCR4 in lymph node metastasis group was 95%（P〈0.05）,and that of VEGF-C was 93%（P〈0.01）.With significant statistical differences compared with no metastasis group.The positive expression of VEGF and CXCR4 were correlated with the tumor of invasion,differentiation and Dukes′stage,and the expression of VEGF was positively correlated with CXCR4.Conclusion There is correlation between the expression of VEGF and CXCR4 in the rectal cancer tissues and lymph node（r=1.000,P〈0.01）,suggesting that they may have synergistic effect in the invasion and metastasis of rectal tumor and they can be considered as an important indicator to the lymph node metastasis of rectal tumor.
