异丙酚静脉麻醉辅助胃肠镜检查的临床应用

目的：2003年6月～2004年6月对860例胃镜检查，490例肠镜检查病人辅以使用异丙酚静脉麻醉检查．分析探讨异丙酚应用于内镜检查中的适应人群、最佳剂量、安全性、顺应性和临床意义。方法：本组病例均无胃肠镜检查及异丙酚麻醉禁忌证，术前与患者签定知情同意书．开通静脉通道。备好各种抢救物品及药品。作好BP、HR、SpO2监护，并作记录。结果：注射异丙酚后，起效时间为15～20s，用药量在100～200mg，检查后对操作过程完全遗忘100％，满意100％。围内镜检查期，BP、HR、SPO2基本维持在正常范围，胃肠镜操作比患者清醒时检查更为顺利。结论：应用异丙酚静脉麻醉辅助内镜检查显著改善病人耐受性，也具有较好的安全性和顺应性，值得推广应用。

Clinical Application of Intravenous Propofo in Gastroscopy

Objective: Gastrocopy is one of the most important diagnosis and treat in digestive department. During the gastrocopy, patients always feel sick, afraid and painful, so it certainly adds some difficulty to the doctors' operation, diagnose and treatment. From Jun. 2003 to Jun. 2004 our hospital succeeds in operating 860 examples of gastrocopy and 490 examples of enteroscopy with the Propofol intravenous anesthesia. I analyze and probe into the suitable patients, the right dosage, safety, compliance and clinical meaning with Propofol used in endoscope examination in this article. Methods: There's no contraindication in gastrocopy, enteroscopy and Propofol anesthesia in all these examples. Make an agreement with the patients before operation. While the patients are having intravenous drip, doctors should prepare all kinds of rescues thing and medicines, watch BP, HR, SpO_2 carefully and keep a record. Results: After injection Propofol the effect lasts 15-20s, the dosage is 100-200mg. 100% of the patients totally forget the course of operation and 100% of the patients are satisfied with it after the examination. During gastrocopy BP, HR, SpO_2 are around the normal range. The operation of gastroocopy and enteroscopy are much easier than they are awake. Conclusion:With the help of Propofol intravenous anesthesia the patients don't feel so bad as they are awake. What's more, it is more safety and compliance. So it is one of the ways worth using widely.
