摘    要:目的 比较国内常规肌内注射法与国外Z径路(Z-track)肌内注射法所致药液回渗情况。方法 2005年5月~2005年12月期间接受肌内注射治疗的患者400例,按年龄分为中青年组200例,老年组200例,每组男女各占一半,采用自身对照,各组每例分别在两侧臀大肌交替使用常规肌内注射法和Z-track肌内注射法给药,观察并记录注射后药液回渗情况。结果 无论在中青年组还是老年组,Z-track肌内注射法均较常规肌内注射法药液回渗的发生率低,差异具有显著性,老年组的差别更加明显。结论 Z-track肌内注射法较常规肌内注射法所致药液回渗的发生率低,相关并发症少,尤其适合在老年人等特殊人群中应用。

Abstract:Objective To make a comparative study of two intramuscular depot injection techniques commonly used at home and abroad in order to identify the preferable technique to be used in clinical nursing practice. Methods Four hundred in-patients who needed intramuscular injection were randomly recruited and evenly divided into two groups young and middle aged group and aged group with equal gender distribution in each group. Every subject received routine and Z-track intramuscular depot injection on alternate day at opposite side of gluteal muscles. Injection related extravasation or seepage was observed and registered, Results In- verse leakage from Z-track maneuver was less frequent than that of routine technique in both groups, The difference was more significant in the aged group, Conclusion Z-track intramuscular injection is superior than the routine intramuscular injection with less seepage complication, It is suggested that this technique should were widely used especially for aged patients.
