急性脑梗死患者血清钾离子浓度变化及其临床意义

摘    要:目的:探讨急性脑梗死患者血清钾离子浓度变化及其临床意义。方法:选取40例急性脑梗死患者为治疗组,同期体检健康者40例为对照组,检测治疗组入院时及治疗2周后血清钾离子浓度,并与对照组进行比较;同时对治疗组入院时、治疗1周和2周时进行神经功能评分。结果:(1)治疗组入院时钾离子浓度明显低于对照组(P<0.05),治疗2周后血清钾浓度升高,但仍低于对照组(P<0.05);(2)治疗组中丘脑病变者血钾浓度下降最明显(P<0.05);(3)治疗组神经功能评分显示与血钾浓度变化有明显相关性(r=0.91,P<0.05)。结论:血清钾离子浓度变化与急性脑梗死患者病变部位有关,丘脑病变者最容易出现低血钾;急性脑梗死患者神经功能恢复与血清钾离子浓度有一定相关性,低血钾患者神经功能恢复差于血钾正常者,早期补钾对急性脑梗死的治疗及预后具有重要临床意义。

Abstract:Objective:To discuss serum potassium ion concentration changes in patients with acute cerebral infarction and their clinical significance.Methods:Select 40 cases of acute cerebral infarction as the treatment group,40 cases of the medical health as control group,detect serum potassium ion concentration of the treatment group in 1th day and after 2 weeks,then compared with the control group;at the same time detect the NDS of the treatment group in 1th day,after 1 week and 2 weeks.Results:(1)Potassium ion concentration of the treatment group was significantly lower than that in control group(P<0.05),after the treatment for 2 weeks it increased,but was still lower than that of the control group(P<0.05);(2)in patients with cerebral infarction in thalamus,potassium concentration was the lowest(P<0.05);(3)NDS had obvious correlation with the changes of serum potassium ion concentration(r=0.91,P<0.05).Conclusion:Serum potassium ion concentration changes have the correlation with the location of acute cerebral infarction,with relevant pathological changes,the patients with cerebral infarction in thalamus is mostly prone to hypokalemia;neurological recoveries in patients with acute cerebral infarction have certain relevance with serum potassium ion concentration,neurological recoveries in patients with hypokalemia are poorer than those in normal,it is important for therapy and prognosis to fill potassium early on patients with acute cerebral infarction.
