喉癌的螺旋CT诊断

摘    要:目的探讨喉癌的螺旋CT表现和价值。方法回顾性分析经手术或活检病理证实的38例喉癌的螺旋CT资料。结果38例喉癌包括声门癌16例,CT扫描见单侧或双侧声带增厚或呈结节状肿物,也见前后联合、环勺间隙受侵;声门上癌5例,表现为病变侵犯单侧或双侧喉室、假声带或会厌前间隙与喉旁间隙,会厌软骨喉面可不规则;声门下癌3例,表现为声门下软组织局限性增厚或肿块,突向气管内;跨声门癌14例,表现为较大组织肿物跨越喉室生长,喉前皮腔受压变窄,环状软骨、勺状软骨破坏。9例有颈淋巴结转移。结论螺旋CT对判断喉癌病灶的部位与侵犯范围有重要价值,但在诊断早期喉癌或向周围侵犯程度较轻时有一定的局限性。

Abstract:Objective To probe manifestations of laryngeal cancer on CT. Methods 38 patients that had been proven laryngeal cancer surgically or pathologically were retrospectively reviewed with their CT imaging data. Results 16 patients were glottic carcinoma showing lateral or bilateral thickened or nodular vocal cord and/or involvement of anterior and posterior symphysis and of space between cricoid cartilage and spooncartilage, 5 cases were supraglottic carcinoma showing involvement of literal or bilateral ventriculus larynges, pseudo-vocal cord or anterior space of epiglottis and space beside throat, and presenting irregular surface of epiglottic cartilage. 3 cases were subglottic carcinoma showing localized thickened tissue or mass protruding to trachea. 14 cases were transglottic carcinoma showing extension of mass across ventriculus larynges, narrowing cavity anterior to throat, and damaging cricoid cartilage and spooncartilage. 9 cases had metastasis to lymph nodes in neck. Conclusion The spiral CT scan is very useful in identifying location and scope of laryngeal cancer however it is inferior in detecting early laryngeal cancer and minor involvement.
