布托啡诺联合吗啡用于术后皮下自控镇痛

目的:比较布托啡诺联合吗啡用于术后病人皮下自控镇痛(PCSA)的临床效果。方法:90例ASAⅠ或Ⅱ级手术病人随机分为布托啡诺组、吗啡组、布托啡诺+吗啡组,术后实施PCSA。采用视觉模拟评分法(VAS)和Ramsay法评价PCSA开始后4、8、16、24、36、48 h镇痛、镇静程度、给药次数及不良反应情况。结果:吗啡镇痛效果好,镇痛满意率高(P<0.01或P<0.05),但不良反应多;布托啡诺+吗啡镇痛、镇静效果满意,恶心、呕吐、头晕、皮肤瘙痒等不良反应的发生率较低。结论:布托啡诺联合吗啡用于PCSA术后镇痛效果满意。

Application of Patient-controlled Subcutaneous Analgesia with Butorphanol and Morphine Postoperation

Objective To investigate the clinical efficacy of patient-controlled subcutaneous analgesia（PCSA）with butorphanol and morphine postoperation.Methods Ninty patients of ASA classⅠor Ⅱ were randomly divided into three groups：butorphanol group,morphine group,and butorphanol combined morphine group,PCSA were all given postoperation.The visual analogue score（VAS） and Ramsay method was used to evaluate the efficacy and degree of analgesia,the times of patient-controlled drug delivery,the adverse reaction at 4,8,16,24,36,48 h after PCSA,respectively.Results The analgesia effect of morphine group was satisfactory but the incidence of adverse reaction was high（P0.01 or P0.05）;the analgesia effect of butorphanol combined morphine group was satisfactory and the incidence of adverse reactions,such as nausea,vomitus,dizziness,pruritus was the least.Conclusion Butorphanol combined morphine could satisfy the needs of PCSA postoperation.
