38例腰椎滑脱的疗效分析

目的探讨腰椎滑脱的技术治疗方法及疗效。方法38例腰椎滑脱患者，术前均有不同程度的腰痛，下肢感觉及运动障碍，及间歇跛行。滑脱部位：L4,5 14例，L5～S124例，采用后路双侧椎板减压，取髂骨植骨，滑脱提拉钉棒复位固定。结果随访时间9个月～4年，平均2年3个月，所有患者腰痛、下肢感觉运动、间歇跛行及性功能障碍均有明显恢复。硬膜破裂3例，修补后未出现脑积液漏，术后半年复查复位丧失10％3倒，所有病例椎间融合良好，无1例发现断钉断棒。结论腰椎滑脱采用后路双侧椎板减压，取髂骨椎体间植骨融合，滑脱提拉钉棒复位固定，不失为一种治疗腰椎滑脱良好方法。

Evaluation in treatment effects of spondylolisthesis in 38 cases

Objective To evaluate the therapeutic effect in the treatment of lumbar spondylolisthesis.Methods Thirty-eight cases of lumbar spondylolisthesis with different degrees of low back pain,numbness and intermittent claudication were included in the study.Among them,there was L4 in 14 cases,L5 in 24 cases.All patients were treated with posterior decompression of total laminectomy and interbody fusion with application of RF pedicle screw spinal system.Results The patients were followed up from 9 months to 4 years with an average of 2.3 years.Both symptoms were significantly relieved in all patients.There were cerebraspinal fluid leakage in 3 cases,loses of correction for 10% were seen in 3 cases at 6 months post operation.All cases achieved a solid union.No complications such as nail or rod breakage were found.Conclusion It is a reliable and effective method to use reduction fixation system combined with canal decompression and intervertebral fusion for the treatment of lumbar spondylolisthesis.
