发蒙法配合子午流注针法治疗神经性耳鸣疗效观察

目的观察发蒙法配合子午流注针法治疗神经性耳鸣的临床疗效。方法 90例神经性耳鸣患者随机分为治疗组、对照组A和对照组B,每组30例,分别采用发蒙法配合子午流注针法、普通针刺、口服中药进行治疗。观察3组治疗前、治疗1个月后、治疗2个月后耳鸣频率、耳鸣响度匹配、耳鸣致残量表(THI)评分的变化,并比较3组临床疗效。结果治疗1个月后,治疗组耳鸣匹配响度较治疗前减小(P<0.05),3组THI评分低于同组治疗前(P<0.01);3组间耳鸣频率、耳鸣响度匹配、THI评分比较,差异无统计学意义(P>0.05)。治疗2个月后,治疗组耳鸣频率低于治疗前(P<0.01);3组耳鸣响度匹配和THI评分与同组治疗前比较,差异有统计学意义(P<0.05,P<0.01);治疗组耳鸣频率明显低于对照组A和对照组B(P<0.01);治疗组耳鸣响度匹配小于对照组A和对照组B(P<0.01);治疗组THI评分低于对照组B(P<0.05),治疗组THI评分与对照组A比较,差异无统计学意义(P>0.05)。治疗组总有效率为90.0%,对照组A为70.0%,对照组B为50.0%,治疗组总有效率优于对照组A和对照组B(P<0.05,P<0.01)。结论发蒙法配合子午流注针法治疗神经性耳鸣平均疗程较普通针刺、口服中药更短,且能够更大程度减轻耳鸣响度、降低耳鸣频率,提高患者生存质量。

Clinical Observations on the Efficacy of Ignorance-enlightening Technique plus Midnight-midday Ebb Flow Acupuncture for Nervous Tinnitus

Objective To observe the clinical efficacy of ignorance-enlightening technique plus midnight-midday ebb flow acupuncture for nervous tinnitus. Method Ninety patients with nervous tinnitus were randomized to treatment, control A and control B groups, with 30 cases in each group and received ignorance-enlightening technique plus midnight-midday ebb flow acupuncture, conventional acupuncture and oral administration of Chinese herbal medicine, respectively. The tinnitus frequency was examined, tinnitus loudness matching was tested and the Tinnitus Handicap Inventory (THI) score was recorded in the three groups before and after one and two months of treatment. The clinical therapeutic effects were compared between the three groups. Result After one month of treatment, the tinnitus loudness matching decreased in the treatment group compared with before (P<0.05), the THI score was lower in the three groups compared with before (P<0.05);there were no statistically significant differences in the tinnitus frequency, tinnitus loudness matching and the THI score among the three groups (P>0.05). After two months of treatment, the tinnitus frequency was lower in the treatment group compared with before (P<0.05);there were statistically significant pre-/post-treatment difference in the tinnitus loudness matching and the THI score in the three groups (P<0.05, P<0.01);the tinnitus frequency and tinnitus loudness were significantly lower in the treatment group than in control A and B groups (P<0.01);the THI score was lower in the treatment group than in control B group (P<0.05);there was no statistically significant difference in the THI score between the treatment and control A groups (P>0.05). The total efficacy rate was 90.0% in the treatment group, 70.0% in control A group and 70.0% in control B group, and the total efficacy rate was better in the treatment group than in control A and B groups (P<0.05, P<0.01). Conclusions The mean course of treatment is shorter in the treatment group than in the other two groups. Ignorance-enlightening technique plus midnight-midday ebb flow acupuncture can reduce tinnitus loudness, decrease tinnitus frequency and improve the patients’ quality of life to a greater extent.
