氟比洛芬酯复合芬太尼用于胆囊术后镇痛的观察

目的:比较氟比洛芬酯复合芬太尼与单纯芬太尼术后静脉镇痛的临床效果.方法:择期行胆囊切除术患者40例,年龄31～67岁.随机分为A(n=20)、B(n=20)两组:A组,芬太尼1.2mg 0.9%NaCl共100ml;B组,氟比洛芬酯100mg 芬太尼0.6mg 0.9%NaCl共100ml.术后采用病人自控静脉镇痛(Patient Control Invenous Analgesia,PCIA).术后48h内行VAS和OVRS疼痛评分,记录PCA泵按压次数及不良反应.结果:术后5、12、24、48h VAS和OVRS两组间无显著性差异(P＞0.05),48h内PCA泵按压次数B组显著高于A组(P=0.046).不良反应中,B组嗜睡发生率显著少于A组(P=0.035),A组呕吐发生率高于B组,但差异不显著(P=0.212).镇痛期间无呼吸抑制.结论:氟比洛芬酯复合芬太尼用于胆囊术后镇痛效果良好,可明显减少芬太尼用量和不良反应.

Application of intravenous flurbiprofen axetil combined with fentanyl for postoperative analgesia in patients undergoing cholecystectomy

Objective To compare analgesic efficacy and complications of intravenous postoperative analgesia with flurbiprofen axetil combined with fentanyl and with fentanyl alone. Methods 40 postoperative patients were randomly divided into two groups with 20 cases each. Fentanyl 1.2mg was used in group A while flurbiprofen axetil 100mg+fentanyl 0.6mg in group B. The drugs in each group were diluted to 100ml with normal saline. Patients received PCIA. The visual analog scale (VAS), observer/verbal ranking scale (OVRS), times of PCA and side effects were recorded during the period of postoperative 48 hours. Results The VAS and OVRS of two groups had no significant difference (P>0.05),times of PCA in group B were significantly higher than those in group A(P=0.046).The incidence of somnolence in group B was significantly less than those in group A(P=0.035).The incidence of Vomiting in group B was less than those in group A, but not significan(P=0.212).No respiratory depression occurred in two groups. Conclusion Intravenous postoperative analgesia with lurbiprofen axetil injection combined with fentanyl has a better analgesic effect than that with fentanyl alone, and can reduce fentanyl requirement and its side effects.
