摘    要:
目的:对比不同剂量布比卡因腰麻用于高原地区患者手术的临床效果及安全性。方法:择期行下腹部、会阴部、下肢手术的患者93例,随机将患者分为低剂量组(59例)、高剂量组(34例),记录麻醉显效时间、麻醉时间、麻醉平面、运动阻滞程度,观察用药后的不良反应。结果:高剂量组患者感觉阻滞起效时间、运动阻滞起效时间显著低于低剂量组患者,而痛觉阻滞持续时间、运动阻滞持续时间显著长于低剂量组;高剂量组患者低血压、心动过缓、恶心呕吐的发生率显著升高。结论:0.75%布比卡因11.25mg腰麻用于高原患者是安全的,其循环系统较稳定,并能达到完善的麻醉效果。

Abstract:
Objective: To compare the efficacy of spinal anesthesia with different dose of bupivacaine in plateau region patients.Methods: 93 patients in plateau region scheduled for lower-abdomen,perineal and lower extremity operations were randomly divided into 2 groups: a small-dose group(59 cases) and a large-dose group(34cases).The change of earlier onset times of anesthesia,anesthesia time,anesthesia level,motor nerve depression and the side effect after medication simultaneously were observed.Results:The onset times of sensory nerve and motor blockade were signifcantly faster in large-dose group than that in small-dose group,but the duration of sensory nerve blockade and duration of motor blockade were significantly longer in large-dose patients than that in small-dose patients.The incidences of hypotension,bradycardia and vomiting in large-dose patients were significantly higher.Conclusion:The spinal anesthesia with 11.25mg of 0.75% bupivacaine for the patients in plateau region is safe and the circulation is stable,which can achieve the better block goal.
