基层医院早期开展腹腔镜胆囊切除术体会

目的探讨早期开展腹腔镜胆囊切除术并发症的防治。方法对我院早期开展腹腔镜胆囊切除术100例进行回顾分析。结果发生并发症10例,其中胃肠道损伤2例,腹腔出血4例,腹壁脓肿1例,气腹综合症1例,胆漏2例。结论基层医院早期开展腹腔镜胆囊切除术,应严格掌握适应证,筛选简单易行病例,固定手术组人员,严格培训,规范操作,熟练技巧,适时中转开腹,能有效降低手术并发症。

The experience of the prevention and treatment of complications after early laparoscopic cholecystectomy in basic hospital

Objective To discuss the prevention and treatment of complications after early laparoscopic cholecystectomy(LC) in basic hospital. Methods Retrospectively analyzed the clinical data of 100 patients underwent LC, and summarize the category, treatment methods and preventive measures of the complications. Results 10 complications occured, including 2 cases of gastrointestinal tract injury, 4 cases of abdominal cavity bieeding, 1 case of mural abscess, 1 cases of aeroperitoneum syndrome, 2 cases of bile leakage. Conclusions Laparoscopic cholecystectomy in basic unit hospital must have strictly mastery of operative indications,sieving simple case,retaining surgeons,strict training,specific standard operation,regulation of operation skills,and timely conversion to laparotomy are the key factors to reduce the complications of LC.
