C-反应蛋白和抗心磷脂抗体与不稳定型心绞痛的关系

摘    要:目的 探讨C-反应蛋白(CRP)和抗心磷脂抗体(ACA)对不稳定型心绞痛(UA)的临床应用价值。方法对116例UA患者分别测定CRP、心肌肌钙蛋白I(cTnI)、肌酸激酶同工酶(CK—MB)、ACA。结果CRP升高67例(57.8%),cTnI升高19例(16.4%),CK—MB升高21例(18.1%),ACA阳性45例(38.8%)。6个月中发生急性心肌梗死(AMI)15例,心源性猝死3例。其中CRP升高组和ACA阳性组AMI的发生率明显高于正常组(P<0.01)。多因素Logistic回归分析ClIP升高、ACA阳性、cTnI升高、陈1日性心肌梗死和胸痛次数是预测近期AMI和心源性猝死的独立危险因素。结论CRP和ACA对UA患者的近期预后与早期治疗有重要的临床价值,可作为UA患者的常规检测指标。

Abstract:Objective To evaluate the clinical significance of C-reactive protein (CRP) and anticardiolipin antibody (ACA) in unstable angina (UA).Methods CRP,cardiac troponin I (cTnI),creatine kinase isoenzymes (CK-MB) and ACA of 116 UA cases were detected.Results CRP was increased in 67 cases (57.8%),cTnI in 19 cases (16.4%),CK-MB in 21 cases (18.1%) and ACA positive in 45 cases (38.8%).Within 6 months,there were 15 cases suffering from acute myocardial infarction (AMI) and 3 from cardiac death.The incidence rate of AMI in CRP elevated group and ACA positive group was significantly higher than in normal group (P<0.01).Multifactor logistic analysis showed that the increase of CRP,cTnI,ACA positive,old myocardial infarction and chest pain times were the independent risk factors predicting AMI and cardiac death.Conclusion CRP and ACA have the important clinical value to the short-term prognosis and early treatment of unstable angina,which can be used as the routine detection indexes for UA patients.
