脑卒中疾病医保患者住院医疗费用的影响因素分析

目的]探讨脑卒中疾病医保患者住院医疗费用的影响因素,为建立合理的单病种费用控制机制提供理论依据.方法]选择某县2004年度在县、镇级12所医院所有住院治疗的城镇脑卒中疾病医保患者126例,从社会因素与非社会因素9个方面对其住院医疗费用进行分析,组间比较采用t检验及单因素方差分析,用多元逐步回归方法进行多因素分析.结果]脑卒中疾病医保患者住院医疗费用的主要影响因素依次为住院日、年龄、医院级别、收入水平,一甲医院住院费用低于二甲医院,随年龄加大、收入水平的提高和住院日的延长,住院费用增加.结论]控制过高医疗费用的关键在于缩短住院日,实施恰当的诊疗措施.

ANALYSIS OF INFLUENCING FACTORS ON THE HOSPITALIZATION EXPENDITURE OF MEDICAL INSURANT PATIENTS WITH STROKE

Objective]To explore the influencing factors on the hospitalization expenditure of medical insurant patients with stroke and to provide the theoretical bases of single entity expenditure mechanism.Methods]126 medical insurant patients with stroke in 12 hospitals in 2004 were selected.The expenditures of medical insurant patients were analyzed from 9 aspects such as social factors,non-social factors and so on.Group comparisons were carried out with t-test and one-way ANOVA(analysis of variance).The multiple stepwise regression analysis was used to study the factors which affected the medical expense.Results]According to the standardized coefficients,the main factors affected the medical expenditures of medical insurant patients with stroke were the hospitalization days,age,nosocomial grade and income level in turn.The medical expenditures in town hospitals were lower than that in county hospitals.The medical expenditures increased with age,income level elevating and hospitalization days protracting.Conclusion]The keys of controlling medical expenditures are that the hospitalization days should be shorten and proper diagnostic measures and treatments should be performed.
