209例中青年院前猝死及复苏状况回顾分析

目的分析广西南宁市中青年院外猝死度现场复苏成败的原因，以进一步提高现场复苏成功率。方法收集1999年1月至2004年12月经本院急诊科外出急救的209例猝死病历，参照Utstein模式进行资料收集并进行回顾性研究。结果猝死前有心血管病史者占首位，为93例，占44．49％；猝死高发年龄40～49岁，占37．32％，男性多于女性；家中猝死者约占50％；先由目击者进行心肺复苏（CPR）的18例中5例复苏有效，有效率27．78％，呼叫至到迭现场平均间期分别为16．4分钟；总复苏成功率为3．82％（8例）。结论南宁市中青年猝死者以心源性为主，现场复苏成功率低，亟待强化院外猝死发生率的急救干预，特别是在人群中普度以CPR为主的初级救护知识和努力缩短急救反应时间。

Retrospective analysis on 209 cases of pre-hospital sudden death of young and medium age patients and on-spot resuscitation

Objective To analyze clinical characteristics and on-spot resuscitation for young and medium age sudden death in Nanning City,Guangxi Zhuan Autonomous Region in order to improve success rate of on-spot resuscitation for sudden death.Methods A consecutive 209 records of sudden death patients who had received pre-hospital emergency medical care from Jan.1999 to Dec.2004 were collected and a retrospective study was performed according to Utstein style.Results There were 93 cases,accounted for(44.49)% of the patients,who had history of cardiovascular diseases before sudden death which was the first reason.Highest incidence of sudden death occurred in the age group of 40-49 years old which accounted for(37.32)% of all the cases;sudden death occurred more often in males than females at same age group and the events taking place at home accounted for 50% of all the patients.Among the 18 patients who had received cardiopulmonary resuscitation(CPR) by witness 5((27.78)%) were responded effectively to the resuscitation.The average time interval from call to arrival at site was(16.4) minutes.There were totally 8 cases((3.82)%) who responded effectively to on-spot resuscitation.Conclusion Cardiovascular disease is a major risk for sudden death of young and medium age persons in Nanning City;the events occurred most often in the patients with cardiovascular disease,especially in males and the success rate of on-spot resuscitation for sudden death is quite lower.It is necessarily to propagandize the knowledge and the basic procedure of applying primary emergency care with CPR in public.The effort should be made in shorting the emergency response time.
