肾盂癌的影像学表现及其检查方法评价

目的：探讨肾盂癌的影像学表现以提高其诊断正确性。材料与方法：搜集经手术病理证实的12例肾盂癌资料，其中11例作IVP，3例作RP，2例作MRI，12例分别是作CT及US检查。着重分析尿路造影与CT表现。结果：IVP和RP显示肾盂、肾盏内不规则形充盈缺损，肾盏积水。当肿瘤较小时，CT平扫见肾盂、肾盏内软组织肿块；当肿瘤侵犯肾实质时，显示肾盂及肾实质内软组织肿块，病灶密度不均匀，可有坏死液化。增强扫描病灶呈轻中度强化。9例IVP及3例RP显示病灶，11例CT及2例MRI均显示病灶，12例作US者仅6例显示病灶。结论：IVP和CT是诊断肾盂癌的首选检查方法。当肿瘤侵犯肾实质或有远处转移时，CT和MRI明显优于IVP和RP。

Imaging Manifestations of Renipelvic Carcinoma and Evaluation of its Diagnostic Examinations

Purpose:To evaluate the imaging manifestations of carcinoma of renal pelves and imporving the diagnostic accuracy.Materials and Methods:Twelve cases of renipelvic carcinoma proved by surgery and biopsy.IVP was performe in 11,RP in 3,CT and US in 12,MRI in 2.Urography and CT findings were mainly analysed.Results:lrregular filling defect and hydronephrosis was seen in the renal pelvic and calyces on IVP and RP.On CT scan,When the tumor was small,soft tissue mass was seen in the renal pelvic and the calyces.When the tumor involved the parenchyma of the renal,soft tissue mass and non-uniform in density,necrosis and fluidzation in the lesion was seen in the renal pelvic and the parenchyma of the renal.On the enhanced CT scan,enhancement from slight to moderate was found in the lesion.The lesion was seen with IVP in 9 cases.RP in 3 cases. CT in 12 cases.MRI in 2 cases.US in 6 cases.Conclusion:IVP and CT is the first method of choice to diagnose the renipelvic carcinoma.When the tumor involving the parenchyma of the renal and metastasis to other,CT and MRI were superior to IVP and RP.
