改良胃空肠Roux-en-Y吻合术在胃肠手术中的应用价值

目的探讨改良胃空肠Roux-en-Y吻合术在胃肠道手术中的应用价值。方法选取2009年10月至2012年6月期间在我院行胃肠道手术的患者64例,随机分为观察组和对照组各32例,观察组患者实施改良胃空肠Roux-en-Y吻合术,对照组患者实施传统标准胃空肠Roux-en-Y吻合术,观察比较两组患者手术时间、术中出血量以及术后吻合口瘘、出血、Roux-en-Y滞留综合征以及碱性反流性胃炎等并发症的发生情况。结果观察组患者手术时间和术中出血量显著少于对照组,差异具有统计学意义（P〈0.05）;术后进行10~24个月随访,观察组患者的术后吻合口瘘、出血、Roux-en-Y滞留综合征以及碱性反流性胃炎等并发症的发生率为12.5%（4/32）,显著少于对照组的34.4%（11/32）,差异具有统计学意义（P〈0.05）。结论改良胃空肠Roux-en-Y吻合术操作简单,手术时间和术中出血量显著减少,且术后并发症少,值得在临床推广与应用。

Value of improved gastrojejunostomy Roux-en-Y anastomosis in gastrointestinal surgery.

Objective To observe the value of improved gastrojejunostomy Roux-en-Y anastomosis in gastro- intestinal surgery. Methods Sixty-four patients cases which needed gastrointestinal surgery were selected from Octo- ber 2009 to June 2012. They were randomly divided into observation group （32 cases） and the control group （32 cases）. The observation group was given improved gastrojejunostomy Roux-en-Y anastomosis surgery, while the control group was given traditional standard gastrojejunostomy Roux-en-Y anastomosis surgery. The operative time, blood loss, and anastomotic leakage, bleeding, Roux-en-Y syndrome and alkaline reflux gastritis incidence of complications were com- pared between the two groups. Results The operative time and blood loss of the observation group were significantly less than those in the control group （P〈0.05）; aider postoperative 10 to 24 months follow-up, the operation anastomotic leakage, bleeding, Roux-en-Y syndrome and alkaline reflux gastritis incidence of complication rate was 12.5% （4/32）, significantly less than 34.4% （11/32） of the control group （P〈0.05）. Conclusion Improved gastrojejunostomy Roux-en-Y anastomosis is simple, with the less operative time, blood loss, and less postoperative complications.
