转铁蛋白受体评估铁过量状况最佳临界值的研究

目的:比较中国成年男性铁过量人群与铁营养状况正常人群血浆可溶性转铁蛋白受体(sTfR)水平,并确定sTfR在评价铁过量时的适宜临界值。方法:从2002年中国居民营养与健康状况调查采集的静脉血液样本中随机抽取226名铁营养状况正常的成年男性作为正常对照组,331名铁过量的成年男性作为铁过量组,比较两组人群sTfR水平,采用工作特征曲线(ROC)法探讨其评估铁过量状况的最佳临界值。结果:铁过量组与正常对照组血浆sTfR浓度分别为1325.46±484.92ng/ml、1788.56±457.02ng/ml,铁过量组sTfR水平显著低于正常对照组(P<0.01)。采用ROC曲线法确定sTfR判断铁过量的最佳临界点为1300ng/ml,对应的敏感度为76.99%,特异度为72.82%。结论:我国成年男性铁过量人群血浆sTfR水平低于铁营养状况正常人群,sTfR<1300ng/ml为评估铁过量的诊断标准。

STUDIES ON THE CRITICAL VALUE OF sTfR FOR ASSESSING IRON OVERLOAD IN CHINESE ADULT MEN

Objective:To study the critical value of soluble transferring receptor (sTfR) for assessing iron overload in Chinese adult men. Method:226 normal iron status and 331 iron overload men were drawn from National Nutrition and Health Survey 2002 as control group and case group respectively. To compare the sTfR concentration and distribution between two groups and ascertain the sTfR critical value to assess iron overload by ROC curve. Results:The concentrations of sTfR were 1 325.46±484.92 ng/ml, 1 788.56± 457.02 ng/ml in case group and control group respectively, the former lower than the latter (P<0.01). The optimal critical value of sTfR to assess iron overload was 1 300 ng/ml, and the corresponding sensitivity and specificity were 76.99%, 72.82% respectively. Conclusion:sTfR concentration was lower in iron overload group than that in normal iron status group. The critical value of sTfR to diagnose iron overload was sTfR <1 300ng/ml.
