手转胎头术纠正枕位异常临床分析

摘    要:目的:探讨手转胎头术纠正枕位异常(持续性枕横位、持续性枕后位)的临床效果。方法:回顾性分析在本院住院分娩.采用手转胎头术纠正枕位异常108例的临床资料。结果:108例中转位成功101例,失败7例,成功率93.5%,无胎儿头皮损伤及颅内出血:无胎盘早剥、脐带脱垂及软产道损伤等并发症。结论:经阴道手转胎头术纠正头位难产枕位异常,具有成功率高、安全、无不良反应、容易掌握等优点,是降低头位难产发生率的有效措施。

Abstract:Objective:To investigate the clinical effect of manual rotation of fetal head on the modification of abnormal occiput position in dystocia, including persistent occiput transverse position and persistent occiput posterior position. Methods:108 patients who delivered by manual rotation of fetal head to adjust the abnormal occiput position in dystocia between June 1,2005 and May 31,2006 in our hospital were retrospectively analysed.Results:101 of the 108 patients succeeded in the manual rotation of fetal head, and 7 patients failed in the modification. The rate of success was 93.5%.None of fetuses had the following complications,including scalp injury,intracranial hemorrhage,placental abruption,prolapse of umbilical cord or laceration of the lower genital tract.Conclusion:Transvaginal manual rotation of fetal head on the modification of abnormal occiput position is considered as an effective method to down regulate the rate of dystocia resulting from fetal head position,which could be carried without adverse reaction. And the technology is safe,easily to master,and has a high achievement rate.
