吻合器直肠黏膜环切术加柱状结扎治疗直肠黏膜内脱垂的临床研究

目的对比三种术式治疗直肠黏膜内脱垂的临床疗效，探讨吻合器直肠黏膜环切术（PPH）加直肠黏膜松弛柱状结扎术治疗直肠黏膜内脱垂的优越性。方法直肠黏膜内脱垂患者130例分为三组，A组43例采用吻合器直肠黏膜环切术加直肠黏膜柱状松弛柱状结扎术，B组30例采用吻合器直肠黏膜环切术，C组32例采用直肠黏膜松弛柱状结扎术。对三组疗效及复发率作比较。结果三组术后有效率比较差异无统计学意义，A组痊愈率显著高于B、C两组，术后随访6-12个月，B、C两组复发率显著高于A组。结论对于直肠黏膜内脱垂所致的便秘及合并直肠前突所致的便秘，吻合器直肠黏膜环切术（PPH）加直肠黏膜松弛柱状结扎术治疗具有疗效好，复发率低等优点。

Clinical Study of Rectal Mueosa Prolapse Treated with Procedure for Prolapse and Hemor- rhoids and the Ligation of Rectal Mucosa

Objective The main aim of this study was to comparative with three mixed nemorrnola surgery treat- ment in the rectal mucosa prolapse clinical efficacy, to explore the reliability of PPH（ procedure for prolapsed and hemorrhoid） and the ligation of rectal mucosa in the treatment of outlet obstructive constipation induced by prolapse of rectal mucosa. Methods A group of 43 cases with stapling hemorrhoids mucosa fibrotomy on rectal mucosa co- lumnar flabby ligation, B group of 30 cases using stapling hemorrhoids mucosa fibrotomy, relaxation C group of 32 cases with rectal mucosa columnar ligation. Results Three groups there was no significant difference in effectiveness comparison, cure rate of group A was significantly higher than group B, C group, followed up for 6 to 12 months postoperatively, B, C, groups of the recurrence rate is induced constipation rectal mucosa and Mergers rectal significantly higher than group A. Conclusion For relaxation sudden induced constipation, PPH and the rectal mucosa re- laxation columnar ligation will be small trauma, easy to operate, fewer complications, short recovery time, high se- curity.
