不同浓度舒芬太尼在下肢骨科手术后硬膜外自控镇痛中镇痛效果的观察

目的:观察不同浓度舒芬太尼在下肢骨科手术后硬膜外自控镇痛中的镇痛效果。方法:将60例下肢骨科手术患者随机分为两组,Ⅰ组术后镇痛用舒芬太尼0.5μg/ml复合0.125%罗哌卡因,Ⅱ组术后镇痛用舒芬太尼0.75μg/ml复合0.125%罗哌卡因,比较两组疗效。结果:两组患者镇痛效果比较,Ⅰ组在术后2 h4、h、8 h、16h的镇痛效果明显优于Ⅱ组(P0.05);镇静效果比较Ⅱ组的镇静评分在术后4 h、8 h高于Ⅰ组,有统计学差异(P0.05);镇痛期间的不良反应的比较无统计学差异(P0.05)。结论:0.5μg/ml舒芬太尼复合0.125%罗哌卡因用于下肢骨科手术硬膜外自控镇痛,取得较满意效果和较少不良反应。

Observation of Effect of Different Concentration Sufentanil in Patient Self-controlled Epidural Analgesia after Operation of Lower Extremities in Orthopaedics

Objective：To observe the effect of different concentration sufentanil in patient self-controlled epidural analgesia（PCEA） after operation of lower extremities in orthopaedics. Method：60 patients with operation of lower extremities in orthopaedics were divided into two groups randomly. The patients received sufentanil 0.5 μg/ml add 0. 125% ropivacaine for analgesia in I group,and sufentanil 0.75 μg/ml add 0. 125 ~ ropivacaine for analgesia in II group. The effect was compared between the two groups. Results：The analgesia effect was better in 2 h,4 h,8 h,16 h after operation in I group than in II group（P〈0.05）. The sedation scores was higher in 4 h and 8 h after operation in II group than in I group, and there was a statistical difference（P~0, 05）. There was no statistical difference（P〉0.05） about the adverse reaction in analgesia period in the two groups. Conclusion：Sufentanil 0.5 μg/ ml add 0. 125% ropivacaine for analgesia has a satisfactory effect and less adverse reaction in operation of lower extremities in orthopaedics.
