机械通气并发应激性溃疡危险因素的Logistic回归分析

目的:探讨机械通气患者并发应激性溃疡的相关危险因素。方法:回顾我科急诊重症监护病房收治250例机械通气患者,对机械通气患者并发应激性溃疡相关危险因素进行单因素及多因素Logistic回归分析。结果:单因素分析显示机械通气患者并发应激性溃疡的危险因素包括年龄、意识障碍、APACHEⅡ评分、机械通气时间、白蛋白含量、呼吸机相关性肺炎(P0.05)。多因素Logistic回归分析显示意识障碍、APACHEⅡ评分、机械通气时间是机械通气患者并发应激性溃疡的独立危险因素(P0.05)。结论:对于有意识障碍、高APACHEⅡ评分以及可能需要长时间机械通气的患者要引起重视,及早地进行应激性溃疡的预防性治疗以及尽量缩短机械通气时间。

Logistic regression analysis on risk factors of mechanical ventilation complicated with stress ulcer

Objective： To explore the related risk factors of mechanically ventilated patients complicated with stress ulcer. Method：We reviewed 250 mechanically ventilated patients in EICU of our department and performed the univariate and multivariate Logistic regression analysis on related risk factors of mechanically ventilated patients complicated with stress ulcer. Result： Univariate analysis showed that risk factors for mechanically ventilated patients complicated with stress ulcer included age, disturbance of consciousness, APACHE II score, duration of mechanical ventilation,albumin, ventilator-associated pneumonia （P~ 0.05）. Logistic regression analysis showed that consciousness, APACHE II score, duration of mechanical ventilation were independent risk factors of mechanically ventilated patients complicated with stress ulcer （P%0.05）. Conclusion： For patients with disturbance of consciousness,high APACHE II score and prolonged mechanical ventilation that may be required,we should pay attention to them and give early preventive treatment of stress ulcer and try to shorten the duration of mechanical ventilation.
