乳腺后间隙麻醉在乳腺肿块微创旋切术中的临床价值

目的研究乳腺后间隙麻醉法在乳腺肿块微创旋切术中的临床价值。方法选取2012年1月~2014年1月期间我院接收治疗的单乳乳腺肿块患者108例,所有患者均采用微创旋切术进行治疗。采用随机数表法将52例单发肿块患者和56例多发肿块患者分为观察组和对照组。观察组采用乳腺后间隙麻醉法进行麻醉,对照组采用传统局部麻醉法进行麻醉。对比两组患者术中麻醉药的用量、止痛效果、手术时间、麻药不良反应等指标。结果单乳单个肿物及单乳多个肿物的观察组患者均较对照组麻醉实施的时间短,使用的麻醉药物较少,手术时间较短,比较两组间差异具有统计学意义(P0.05)。比较两组患者麻醉效果,差异不具有统计学意义(P0.05)。观察组患者术后的总不良反应率为5.56%,显著低于对照组患者的29.63%,比较两组间差异具有统计学意义(P0.05)。结论与传统局部麻醉法比较,乳腺后间隙麻醉法使用的麻药剂量少,手术时间短,且麻醉效果与传统局部麻醉法无显著差异,可以在乳腺肿块微创旋切术中推广使用。

The Clinical Value of Post-mammary Local Anesthesia in Mammotome Minimally Opearation for Breast Tumors

Objective To explore the clinical value of post-mammary local anesthesia in Mammotome minimally opearation for breast tumor patients. Methods There were 108 patients enrolled from January 2012 to January 2014 in our hospital to receive the Mammotome minimally opearation for breast tumors. 52 ca- ses of uni-tumor and 56 cases of multi-tumor were randomized into two groups. In the observe group, the post- mammary local anesthesia was used. And in the control group, the traditional regional anesthesia was used. The intraoperative anesthetic effect, the anesthetic dosage, the operation time and the anesthetic adverse reactions were compared. Results Both in the uni- and multi-tumor patients of the observe group, the anesthesia time, the anesthetic dosage, and the operation time were better than the control group （P 〈 0.05）. The anes- thetic adverse reaction rate of the observe group was 5.56%, which was lower than 29.63% of the control group（P 〈 0.05）. Conclusion It shows shorter anesthesia time and operative time, and less anesthetic dos- age of the post-mammary local anesthesia in Mammotome minimally opearation for breast tumor patien s, which is worthy of promotion.
