基于微信平台的延续护理模式在TIPS术后出院患者中的应用

目的:探讨基于微信平台的延续护理模式(TCM)在经颈静脉肝内门体分流术(TIPS)术后出院患者中的应用效果。方法:选取2018年10月1日~2019年10月1日在山东省某三级甲等综合性医院消化内科住院的行TIPS治疗的70例肝硬化门脉高压患者为研究对象,按照随机数字表法分为对照组和实验组各35例,对照组实施常规健康教育及随访,实验组在常规健康教育及随访基础上采用基于微信平台的TCM;比较两组术后1、3个月的遵医行为、护理满意度、肝性脑病发生情况及再住院率。结果:术后1、3个月,实验组按时用药、饮食控制、大便通畅、定期复诊的依从性均高于对照组(P<0.05,P<0.01),护理满意度均高于对照组(P<0.01),肝性脑病发生率及再住院率均低于对照组(P<0.05,P<0.01)。结论:基于微信平台的TCM可显著提高TIPS术后出院患者的遵医依从性和护理满意度,降低肝性脑病发生率及再住院率,具有临床推广意义。

Application of Transitional Care Model Based on WeChat Platform in Patients Discharged after TIPS Surgery

Objective:To explore the application effect of transitional care model(TCM)based on WeChat platform in patients discharged after transjugular intrahepatic portal-body shunt(TIPS)surgery.Methods:A total of 70 patients with liver cirrhosis portal hypertension after TIPS admitted from October 1,2018 to October 1,2019 in digestive internal medicine department in a Grade 3 Class A general hospital in Shandong province were selected as the research objects.According to random number table method,they were divided into control group and experimental group with 35 cases each.The control group was given routine health education and follow-up,and the experimental group was given TCM on WeChat platform based on the routine health education.The compliance behavior,nursing satisfaction,occurrence of hepatic encephalopathy and readmission rate of the two groups at 1 and 3 months after operation were compared.Results:The compliance of medication on time,diet control,defecate unobstructed,and regular follow-up in the experimental group was higher than that of the control group 1 and 3 months after the operation(P<0.05,P<0.01).The nursing satisfaction was higher than that of the control group(P<0.01),and the incidence of hepatic encephalopathy and the rate of readmission were lower than that of the control group(P<0.05,P<0.01).Conclusion:TCM based on WeChat platform can improve the compliance and nursing satisfaction of patients discharged from the hospital after TIPS surgery,and reduce the incidence of hepatic encephalopathy and the rate of readmission.It has clinical promotion significance.
