腹腔镜下完全腹膜外补片植入术治疗腹股沟疝临床分析

目的：探讨腹腔镜下完全腹膜外补片植入术（TEP）治疗腹股沟疝的临床疗效。方法以2012年12月～2014年1月来我院就诊的90例腹股沟疝患者作为研究对象，对患者实施TEP手术，并对手术技巧经验进行总结。结果90例手术均取得较好的结果，住院时间（4.1±1.2）d,手术时间（59.0±36.3）min,术中出血量（14.3±8.3）mL，恢复天数（8.1±1.3）d；腹膜撕裂发生率为10.00%（9/90）；术后皮下气肿发生率为4.44%（4/90）,腹股沟血清肿发生率为3.30%（3/90），术后未出现持续性神经性疼痛、切口感染及补片感染；复发率1.1%（1/90）。结论 TEP治疗腹股沟疝安全，有效；具有术后疼痛轻，恢复快，复发率低，具有费用低，美容，手术操作简单等特点，值得临床推广应用。

Laparoscopic total extraperitoneal prosthetic for treatment of inguinal hernia

Objective To explore and summarize experiences in laparoscopic total extraperitoneal prosthetic （TEP） for treatment of inguinal hernia.Methods 90 patients with inguinal hernia treated in our hospital from Dec 2012 to Jan 2014 were selected as research objects. All the patients were treated by TEP surgery to summarize surgical techniques and experiences.Results All the 90 cases had successful surgery with hospital stay of （4.1±1.2） days, surgical duration of （59.0±36.3） min, intraoperative blood loss of （14.3±8.3） mL, recovery time of （8.1±1.3） days. The incidence of peritoneal tear, postoperative subcutaneous emphysema and groin seroma was 10.0%（9/90）, 4.44%（4/90）, and 3.30%（3/90） respectively. No persistent neuropathic pain, incision infection or patch infection was observed after the surgery. The recurrence rate was 1.1%（1/90）.Conclusion TEP is safe and effective for treatment of inguinal hernia with mild postoperative pain, fast recovery, low recurrence rate, low cost, cosmetic effect, simple operation and other characteristics, which is worthy of clinical application and promotion.
