剖宫产术后切口妊娠74例临床分析

目的:研究子宫切口妊娠的临床特点,探讨其诊断治疗方法。方法:回顾性分析74例剖宫产切口妊娠患者的病史特点、临床表现、诊断及治疗方法。结果:74例患者中,63例患者经甲氨喋呤杀胚后B超监视下清宫,56例治愈,5例行经腹子宫切口妊娠病灶切除术,2例行子宫全切术;11例行双侧子宫动脉化疗栓塞术,48 h后行清宫术,必要时可行双侧子宫动脉化疗栓塞术,可有效避免子宫切除。结论:临床医生应提高对该病的认识,在治疗过程中,医生应根据患者病情及医院条件,选择适宜的治疗方法,施行个体化的治疗方案。

Clinical analysis of 74 cases of cesarean scar pregnancy

Objective To study the clinical features of uterine incision pregnancy, investigate its diagnosis and treatment.Methods A retrospective analysis of 74 cases of cesarean section in patients with pregnancy characteristics of medical history, clinical manifestations,diagnosis and treatment.Results Of the 74 patients,63 patients were treated by methotrexate killing embryo B ultrasound monitoring after curettage,56 cases were cured,5 cases of abdominal uterine incision pregnancy lesion resection,2 cases underwent total hysterectomy; 11 patients underwent bilateral uterine artery chemotherapy and embolization,48 h after curettage,when necessary feasible bilateral uterine artery chemotherapy embolization,can effectively avoid hysterectomy.Conclusion Clinicians should raise the awareness of the disease,in the course of treatment,the doctor should be according to the conditions of patients and hospital,choosing appropriate methods of treatment,implementation of individualized treatment plan.
