肝素钠、肝素锂抗凝血浆代替血清生化检验的比较研究

摘    要:目的探讨肝素钠、肝素锂抗凝血浆代替血清用于临床生化检验的可行性。方法随机抽取40例患者和健康体检者静脉血标本,将每例标本3ml分别注入普通试管及肝素钠、肝素锂抗凝管中,于同一生化仪上对常规生化项目进行检测,并对结果进行统计学分析。结果Cr、ALT、AST、GGT、TBil、ALB、TBA、P、CHO、CK-MB、AMS、TG、Glu、CL等项目肝素锂抗凝血浆与血清比较,差异无统计学意义(P〉0.05);而BUN、UA、ALP、DBil、TP、Mg、HDL-C、LDL、LDH、CK、K、Na、TCa等项目肝素锂抗凝血浆与血清2组间比较差异有统计学意义(P〈0.05);而BUN、UA、ALP、TBil、DBil、Mg、CHO、LDL、LDH、CK、TG、K、Na、TCa等项目肝素钠抗凝血浆与血清2组间比较差异有统计学意义(P〈0.05)。BUN、Cr、UA、ALT、AST、GGT、ALP、TBil、DBil、TP、ALB、TBA、Mg、CHO、HDL-C、LDL、LDH、CK、CK-MB、AMS、TG、Glu、Cl、Na等24个项目肝素锂与肝素钠抗凝血浆与血清相关性良好(r〉0.800),而P、K、TCa3个项目肝素锂与肝素钠抗凝血浆与血清相关性较差(r〈0.760)。结论大部分常规临床生化检验项目可用肝素锂抗凝血浆代替血清作为标本,但需建立回归方程或血浆参考体系,少数项目不宜用肝素类抗凝血浆。

Abstract:Objective To evaluate the possibility of using sodium-heparin plasma or lithium-heparin plasma samples for measurement of chemistry analysis in stead of serum. Methods The routine biochemical indicators of 40 patients and 40 normal individuals were mmeasured with serum and heparin plasma by the same automatic biochemical analyzer. The testing results were recorded and analyzed comparatively. Results There was statistical difference in BUN. UA. ALP. TBil. DBil. Mg. CHO. LDL. LDH,CK,TG,K,Na,TCa(P 〈0.05) ,but not in Cr,ALT,AST.GGT.TBil.ALB.TBA.P.CHO.CK-MB.AMS.TG.Glu. CI( P 〉 O. 05 ) between result of serum and lithium heparin plasma. The correlation in BUN. Cr. UA. ALT. AST. GGT. ALP. TBil. DBil. TP, ALB, TBA, Mg, CHO, HDL-C, LDL, LDH. CK. CK-MB. AMS. TG. Glu. Cl and Na was good between the result of serum and plasma ( r 〉 0. 800), but not in P, K and TCa ( r 〈 0. 760). Conclusions Heparin plasma can be used to test a lot of routine biochemical projects under the control of regression and normal plasma reference range. The heparin plasma can not stead of serum in some projects detecting.
