糖尿病周围神经病变患者血清抗神经节苷脂抗体的相关研究

摘    要:目的:探讨2型糖尿病周围神经病变(DPN)的发病因素,及与血清抗神经节苷脂抗体(anti-GS-Ab)之间的关系。方法:受试者分为3组:糖尿病无神经病变(DM组)42人,糖尿病神经病变(DPN)组46人,正常对照组(N组)40人,采用固相酶免疫法测定各组anti-GS-IgM-Ab、anti-GS-IgG-Ab。美国Nicolet Viking IV型电图仪测定神经传导速度。结果:DPN患者anti-GS-IgM-Ab、anti-GS-IgG-Ab的阳性率分别为45.65%及23.91%显著高于N组及DM组。DPN组anti-GS-IgM-Ab与周围神经病变的临床分级(DPNC)、糖化血红蛋白(HbAic)和NO呈显著正相关;与病程、超氧化物歧化酶(SOD)呈显著负相关;多元回归分析显示NO、DPNC或SOD对anti-GS-Ab有显著相关。结论:anti-GS-Ab在DPN的病理过程中起重要作用。对DPN的诊断及病情判断有参考价值。

Relationship about Serum Anti-Gangliosides Antibody in Type 2 Diabetes Mellitus Patients with Peripheral Neuropathy

Abstract:To ascertain the relationship between diabetic peripheral neuropathy (DPN) pathogenic factors and serum anti-gangliosides antibody (anti-GS-Ab) in type 2 diabetes mellitus (Type 2 DM). Methods: The receivers were divided into diabetes mellitus with no peripheral neuropathy group (DM group) (n = 42), DPN group (n = 46) and healthy control group (N group)( n = 40). Anti-GS-IgM-Ab and anti-GS-IgG-Ab were detected by solid enzyme immunoassay. The speed of nervous conduct was measured with American Nicolet Viking IV type electromyograph (EMG). Results: The positive rates of anti-GS-IgM-Ab and anti-GS-IgG-Ab in DPN group were 46.74 % and 23.91 % respectively. They were obviously higher than those in N group and DM group. They were significantly positively correlated with DPN clinical classification (DPNC),HbA1c and NO, and significantly negatively correlated with course and SOD. The multiple regression analysis showed that NO, DPNC and SOD had significant effects on anti-GS-Ab. Conclusion: Anti-GS-Ab plays an important role in DPN pathological process. It has a reference value in diagnosis and condition judgment in DPN.
