新生儿机械通气撤机失败原因分析

目的探讨新生儿撤机失败的常见原因，以提高新生儿呼吸机治疗的成功率，降低病死率。方法对62例机械通气的新生儿进行回顾性分析。结果共治愈49例（79．03％），一次撤机成功42例（67．74％），重复上机病死率达53．33％。撤机失败最常见原因是各种肺部并发症，尤其严重肺部感染；其次是未掌握好撤机的适当时机。结论进行机械通气时要严格执行无茵操作，预防肺部感染及其他并发症，掌握好撤机指征，同时撤机后继续加强气道护理，有利于提高撤机的成功率。

REASONS OF EVACUATING FAILURE IN MECHANICAL VENTILATION OF NEWBORNS

Objective To enhance the success ratio of mechanical ventilation and reduce the mortality rate in the newborns,we approached the common reason of failure in evacuating the respirator.Methods Data of 62 newborns with mechanical ventilation was retrospectively analyzed.Results 49 cases(79.03%) were cured,42 cases(67.74%) evacuated the respirator successfully for the first time.The mortality rate reached 53.33%,when the machine was used repeatedly.The most common reason of failure in evacuating the respirator was kinds of lung's complications,especially the serious infection.The second was choosing the bad opportunity to evacuate the machine.Conclusion It must be strict to execute the aseptic manipulation, to prevent the infections of lung and else complications,and to grasp the correct opportunity in evacuating machine When operated on mechanic ventilation,and keep on nurse the gas channel after evacuate machine.These will enhance the success ratio of evacuate the respirator.
