经脐孔腹腔镜辅助小儿阑尾切除术

目的探讨经脐孔腹腔镜辅助小儿阑尾切除术的可行性。方法2005年2月~2006年2月对76例小儿急性阑尾炎、化脓性阑尾炎采用经脐孔腹腔镜辅助小儿阑尾切除术。脐环下、右侧缘2个0.5cm切口,置入2个外径5.5mmtrocar,腹腔镜下找到阑尾,夹住阑尾中段,解除气腹,连接两切口,逐层进腹,将阑尾完整地从脐部拖出腹腔外,采用常规开腹阑尾切除的方法处理阑尾及系膜。结果手术时间15~40min,(20.6±6.4)min;术后排气时间4~18h,(12.0±3.5)h。术后切口全部一期愈合。1例术后1个月出现线头异物反应,换药取出线头后治愈,脐部亦未见明显瘢痕。74例随访2~14个月,(7.3±3.6)月,脐部未见切口瘢痕。结论经脐孔腹腔镜小儿阑尾切除术治疗小儿单纯性、化脓性阑尾炎可行。

Transumbilical Laparoscopic-assisted Appendectomy in Children
