非甾体抗炎药相关性上消化道出血的临床特点

目的 　分析非甾体抗炎药 ( NSAIDs)相关性上消化道出血的临床特点。 方法 　调查我院 1999年 1月～ 2 0 0 3年 12月上消化道出血住院治疗患者的临床资料 ,根据出血前 1周内有无服用 NSAIDs史 ,分为服药组和未服药组两组 ,比较两组病人临床资料。结果 　共有 42 3例患者纳入分析 ,其中服药组 94例 ,未服药组 3 2 9例。服药组年龄高于未服药组年龄 ( P<0 .0 0 1) ;服药组胃溃疡和复合溃疡发病率高于未服药组 ( P<0 .0 0 1) ;但是两组的 Hp菌感染状态、性别差异无显著性 ( P<0 .0 0 5 ) ;在出血方式上无相关性 ( r<0 .4)。结论 　应加强对 NSAIDs相关性上消化道出血的认识 ,尽量减少 NSAIDs的不良反应。

The Clinical Characteristics of Upper Gastrointestinal Hemorrhage Associated with non-steroidal Anti-inflammatory Drugs

OBJECTIVE To study the clinical characteristics of upper gastrointestinal drugs(NSAIDs).METHODS Patients who were hospitalized in our hospital with upper gastrointestinal hemorrhage during the period of Jan,1999 and Dec,2003 were divided into two groups according to with of NSAIDs or without in one week previous to the onset of bleeding.RESULTS There were 423 patients in the study. Among them 94 cases were NSAIDs group and 329 cases were non-NSAIDs group.There was significant difference in age,gastroscope diagnosis (P<0.001)between the above two groups.However,there was no difference between two groups in Helicobacter pylori(Hp)was postive and sex respectively(P<0.005).The pattern of the bleeding was not relevant (r<0.4).CONCLUSION In order to decrease the occurrence of NSAIDs complication, the clinical characteristics of NSAIDs related upper gastrointestinal hemorrhage should be better understood.
