超重/肥胖对儿童肠套叠空气灌肠复位的预后影响:倾向性评分匹配分析

目的:探讨超重/肥胖对儿童肠套叠空气灌肠复位（AER）预后的影响。方法:回顾性分析342例经AER的肠套叠患儿临床资料，根据体重指数（BMI）将其分为超重/肥胖（BMI百分比≥85%）及非超重/肥胖（BMI百分比＜85%）者，进行倾向性评分匹配后，运用单因素和多因素Logistic回归分析总结超重/肥胖对患儿的预后影响。结果:342例患儿经1:1倾向性评分匹配筛选出65例超重/肥胖及65例非超重/肥胖者，两组临床资料的差异无统计学意义（P＞0.05）。匹配后分析显示，超重/肥胖（OR＝19.483，95%CI:4.527~83.857）、白细胞（OR＝1.347，95%CI:1.138~1.594）及血乳酸（OR＝6.072，95%CI:2.149~17.151）为患儿AER预后不良的独立危险因素（均P＜0.05）。结论:超重/肥胖是儿童肠套叠AER不良预后的独立危险因素。

Effect of overweight and obesity on the prognosis of intussusception treated by air enema reduction in children: propensity score matching analysis

Objective: To investigate the effect of overweight and obesity on the prognosis of intussusception treated by air enema reduction（AER） in children. Methods: A retrospective analysis of the clinical data of 342 children with intussusception traeted by AER was performed. According to body mass index（BMI），they were divided into overweight and obese（BMI percentage ≥85%） and non-overweight-and-obese（BMI percentage <85%）. After propensity score matching，univariate and multivariate Logistic regression analysis were used to summarize the effects of overweight and obesity on the prognosis of patients. Results: A total of 65 cases of overweight and obesity and 65 cases of non-overweight and obesity were screened out from 342 children with 1:1 propensity score matching，and there was no significant difference in clinical data between the two groups（P>0.05）. Analysis after matching showed that overweight and obesity （OR=19.483，95%CI:4.527-83.857），white blood cell count（OR=1.347，95%CI:1.138-1.594） and blood lactate （OR=6.072，95%CI:2.149-17.151）were independent risk factors for poor prognosis of children with AER（all P<0.05）. Conclusion: Overweight and obesity are independent risk factors for the poor prognosis of children with intussusception AER.
