颅底沟通性脑膜瘤的MRI及相关特征分析

摘    要:
目的:探讨颅底沟通性脑膜瘤的MRI特征及相关临床特点,提高特殊类型脑膜瘤的认识水平。方法:回顾性分析11例经手术和病理证实的颅底沟通性脑膜瘤的MRI及有关临床资料,全部病例术前均行MRI平扫和Gd-DTPA增强检查,扫描层面包括轴位、冠状位及矢状位。结果:肿瘤形态呈分叶结节状,大小不一,在T1WI和T2WI序列上均呈较均匀等或稍长信号,部分瘤体边缘不清,无坏死、囊变、出血和钙化,增强扫描呈较均匀明显强化;4例伴有颅底解剖腔(孔)变形或扩大,颅底骨质局限性吸收、硬化或破坏共5例;2例术后病理可见肿瘤细胞轻度核分裂像,呈低度恶性。结论:颅底沟通性脑膜瘤有一定的MRI特征,术前明确肿瘤的部位、大小、形态以及周围结构的改变并作出正确诊断有助于制定最佳手术方案和评估预后提供客观可靠信息。

Abstract:
Objective:To study the MRI diagnostic features of skull communicating meningioma and to analyze relative clinical characteristics,and to improve the understanding of skull meningioma.Methods:MRI findings of 11 cases proved by opration and pathology were reviewed retrospectively.All of cases were performed Gd-DTPA enhancement.Results:Size and shape of all 11 cases were different from each other,MRI findings of all tumors showed non-calcified mass and homogeneous with no hyodense cystic areas,presented as homogeneous iso-or slightly hyposignal intensity both on T1WI and T2WI,enhanced remarkable after injecting Gd-DTPA.Anatomy foramen or spaces of skull were changed or enlarged to a certain extent in 4 cases.5 cases were showed osteosclerosis and detruction of bone.2 of 11 cases in pathological slice showed cellular atypia.Conclusion:There are several characteristics on the MRI of skull communicating meningioma. MRI can show the intra-tumor and surrouding structures and the extent of tumor clearly,which are helpful in predciting resectabitity and evaluating the response to surgical treatment.
