腹腔镜辅助经阴道子宫切除术临床效果分析

目的通过腹腔镜辅助经阴道子宫切除术（1aparoscopically assisted vaginal hysterectomy，LAVH）与经腹全子宫切除术（trans-abdominal hysterectomy，TAH）的临床效果比较，分析LAVH的临床价值。方法将宜兴市中医医院2010年10月至2011年8月行LAVH患者36例作为研究组，抽取同期行TAH患者36例作为对照组，将两组患者临床资料进行回顾性分析。结果LAVH组患者术后肛门排气时间及术后住院时间均较TAH组短，术后疼痛程度及术后病率也较TAH组轻，两组比较差异有统计学意义（P〈0．05）；手术时间、术中出血量两组比较差异无统计学意义（P〉0．05）。结论LAVH具备腹腔镜手术及阴道手术两者之优点，具有损伤小、恢复快、切口小、疼痛轻等优点，值得临床推广应用。

Clinical analysis of laparoscopically assisted vaginal hysterectomy

Objective To analysis clinical effect of Laparoscopically assisted vaginal hysterectomy （LAVH） through comparing clinical effect on LAVH with trans - abdominal hysterectomy （TAH）. Methods Clinical data from 36 cases taken with LAVH in study group and another 36 cases taken with TAH in control group in Yixing Hospital of Traditional Chinese Medicine between October 2010 and August 2011 were retrospectively analyzed. Results The anus exhaust time and length of stay in the hospital of LAVH group was respectively shorter than that of TAH group that the difference was significant（ P 〈 O. 05 ）. Postoperative painless degree and postoperative morbidity of LAVH group was significantly lighter than that of TAH group（ P 〈 0. 05 ）. The difference of average operating time and the mean operating blood loss was no significant between two groups （ P 〈 0.05 ）. Conclusion LAVH has good quality of laparoscopic operation and Operation on vagina so that it is worth generation and application.
