亚低温护理对重症颅脑外伤临床疗效及血液流变学的影响

目的观察亚低温护理对重症颅脑创伤(TBI)患者临床疗效及血液流变学的影响。方法将50例TBI患者采用前瞻性随机对照原则分为亚低温护理组(25例)和对照组(25例)。亚低温在对照组常规综合治疗和护理基础上,采用电子降温毯,头部和大血管处放置冰袋方法护理。15 d后观察2组患者神经功能缺损积分、格拉斯哥预后评分及治疗前后血液流变学的改变。结果亚低温组患者治疗15 d后神经功能缺损积分及格拉斯哥预后评分与对照组比较,差异均有统计学意义(P0.05);亚低温组15 d后血液流变有明显改善,与对照组比较,差异均有统计学意义(P0.05)。结论亚低温护理能降低TBI患者的血黏度,能明显改善预后,是临床治疗TBI的重要组成部分,为治疗TBI的疗效观察,提供一定依据。

Effect of sub-hypothermia nursing care on clinical efficacy and hemorheology of patients with severe traumatic brain injury

Objective]To observe the effect of sub-hypothermia nursing care on clinical efficacy and hemorheology of patients with severe traumatic brain injury(TBI).Methods]50 TBI patients were divided into the sub-hypothermia nursing care group(25 cases) and the control group(25 cases) with the prospective randomized principle.Based on the conventional treatment and nursing care of the control group,the sub-hypothermia nursing care group was given electric cooling blanket and placing the ice bag on the head or big vessels.After 15 days of treatment,the nerve function deficit score,Glasgow outcome scale,and changes of hemorheology before and after treatment in patients of two groups were observed.Results]After 15 days of treatment,there were significant differences in the nerve function deficit score and Glasgow outcome scale between the sub-hypothermia nursing care group and the control group(P<0.05),and the hemorheology indexes in the sub-hypothermia nursing care group had improved significantly as compared to those in the control group(P<0.05).Conclusion]The sub-hypothermia nursing care can decline the blood viscosity of TBI patients and obviously improve the prognosis.It is the important part of clinical treatment,which provides the basis for the observation of TBI treatment.
