胺碘酮治疗老年慢性心衰合并心律失常效果观察

目的：观察胺碘酮对老年慢性心衰合并室性心律失常患者的治疗效果。方法将2012年10月—2013年10月第四军医大学西京医院收治的老年慢性心律衰竭并室性心律失常患者72例按照治疗方法分为实验组和对照组，每组36例，对照组给予常规抗心衰治疗，实验组在对照组基础上加用胺碘酮治疗。治疗4周后，比较两组心功能和心律失常改善情况、治疗前后两组QTc、心率变化及不良反应发生率。结果实验组心功能改善和心律失常改善情况均优于对照组(P<0．01)；两组治疗后QTc和心率较治疗前显著改善(P<0．01)，治疗后实验组QTc和心率改善优于对照组(P<0．01)；实验组不良反应发生率显著低于对照组(P<0．05)。结论胺碘酮治疗老年心衰合并室性心律失常患者效果明显，可很好地改善临床症状，安全性高。

Effect Observation of Amiodarone in Treatment of Elderly Patients with Chronic Heart Failure and Cardiac Ar-rhythmias

Objective To observe the effect of Amiodarone in treatment of elderly patients with chronic heart failure and cardiac arrhythmias. Methods A total of 72 elderly patients with chronic cardiac failure and cardiac arrhyth-mias admitted during October 2012 and October 2013 were divided into experimental group ( n=36 ) and control group (n=36) according to therapeutic methods. The control group underwent conventional anti-heart failure therapy, while the experimental group was treated with Amiodarone based on the control group's therapy. After treatment for 4 weeks, the improved conditions of heart function and arrhythmia were compared, and the changes of Q-T interval corrected for heart rate ( QTc) and heart rate, and the incidence rate of complications were also compared in the two groups before and after the treatment. Results The improved conditions of heart function and arrhythmia in the experimental group were superior to that in the control group (P<0. 05);after the treatment, the QTc and heart rate were better improved, compared with those before the treatment in the two groups (P<0. 01), and the improvement conditions in the experimental group were better than those in the control group (P<0. 01). The incidence rate of complications in the experimental group was low-er than that in the control group (P<0. 05). Conclusion Amiodarone in treatment of elderly patients with chronic heart failure and cardiac arrhythmias has good therapeutic effect and good improvement of clinical symptoms with good safety.
