甲状腺腺瘤和结节性甲状腺肿Ang-1的表达及其与超声造影定量参数的关系

摘    要:目的:研究甲状腺腺瘤和结节性甲状腺肿中血管生成素-1(angiopoietin-1,Ang-1)的表达情况,并探讨其与超声造影的相关性.方法:回顾性分析40例甲状腺腺瘤和40例结节性甲状腺肿患者的超声造影检查结果,根据时间强度曲线获得造影参数.术后标本用Western blot方法检测其Ang-1表达情况,并与超声造影指标进行相关性分析.结果:40例甲状腺腺瘤36例造影后肿块与周围甲状腺表现为高增强,4例为等增强.其中34例肿块内部增强均匀.40例结节性甲状腺肿患者中30例造影后结节与周围甲状腺实质等增强,6例出现高增强,4例为低增强,其中14例患者肿块内部回声不均匀.甲状腺腺瘤组mTT、IMAX 大于结节性甲状腺肿组(P<0.05);甲状腺腺瘤较结节性甲状腺肿Ang-1表达明显升高(P<0.01).Ang-1表达水平与RT、TTP、mTT呈负相关(P<0.05),与IMAX呈正相关(P<0.05).结论:Ang-1在甲状腺腺瘤中的表达水平高于结节性甲状腺肿,并与超声造影指标存在相关性,超声造影可为二者的诊断及预后提供依据.

Abstract:Objective:To investigate the Ang-1 levels in thyroid adenoma and nodular goiter,and the relationship between Ang-1 levels and imaging parameters of ultrasound contrast.Methods:Total of 40 cases of thyroid adenoma and 40 cases of nodular goiter patients were analyzed with ultrasound contrast retrospectively.Imaging parameters were got by time intensity curves.The Ang-1 expression in thyroid specimens was analyzed by Western blot,and performed correlation analysis with the imaging parameters of ultrasound contrast.Results:Of the 40 cases 36 with thyroid adenoma were hyper echo,4 cases were equal echo compared with adjacent normal tissues,among them,34 cases got homogeneous enhancement.30 of the 40 cases with nodular goiter were equal echo,6 cases were hyper echo and 4 cases hypo echo compared with adjacent normal tissues.Inhomogeneous enhancement was found in 14 cases.The levels of mTT and IMAX in thyroid adenoma were higher than that of nodular goiter (P<0.05).More Ang-1 expressed in thyroid adenoma than that in nodular goiter (P<0.01).The levels of Ang-1 were negatively correlated with RT,TTP,mTT (P<0.05),and positively correlated with IMAX (P<0.05).Conclusion:Ang-1 was higher expressed in thyroid adenoma than in nodular goiter.There was an correlation ship between imaging parameters of ultrasound contrast and Ang-1 levels.It may provide evidences for the diagnosis and prognosis between them.
