电视辅助胸腔镜肺叶切除术治疗非小细胞肺癌的疗效观察

目的 比较电视胸腔镜下肺叶切除与开胸肺叶切除治疗早期非小细胞肺癌的临床效果.方法 收集81例初诊为早期非小细胞肺癌实施手术治疗的患者为研究对象,并将其分为2组:观察组和对照组.观察组给予胸腔镜肺叶切除术,对照组给予开胸肺叶切除术.比较2组患者手术时间、术中出血、术后拔管时间、住院时间及术后并发症情况.定期随访,比较患者术后3个月、6个月、12个月及3年的生活质量及3年生存率.结果 2组患者的手术时间、拔管时间无明显差异(P>0.05),但观察组患者术中出血及住院时间明显低于对照组(P<0.05).观察组患者有7例出现肺部感染,并发症发生率17.1%;对照组患者并发症合计16例,并发症发生率为40.0%.观察组并发症发生率明显低于对照组(P=0.022).术后第3个月、6个月观察组患者的生存质量明显高于对照组(P<0.05),术后第12个月2组患者的生存质量无明显差异(P>0.05).观察组3年生存率为29.3%,对照组的生存率为30.0%,差异无统计学意义(P=0.994).结论 电视胸腔镜下肺叶切除术治疗非小细胞肺癌,手术创伤小,术中出血少,术后并发症少,术后生存质量高,术后3年生存率与开胸肺叶切除术无明显差异,近期效果满意.

Efficacy of Vdeo-assisted Thoracic Lung Resection for Non-small Cell Lung Cancer

Objective To compare the curative effect between video-assisted thoracic lung resection and thoracotomy for non-small cell lung cancer. Methods 81 cases first diagnosed as early non-small cell lung cancer patients and underwent surgi-cal treatment were selected as the research object. They were divided into the observation group and the control group. The obser-vation group was given thoracoscope lung resection, the control group was given thoracotomy lung resection. Compare operation time,intraoperative bleeding,postoperative extubation time and length of hospital stay,and postoperative complications of the 2 groups. Postoperative 3-month,6-month,12-month and 3-year life quality and 3-year survival rates were evaluated. Results The operation time and postoperative extubation time of the 2 groups of patients had no significant difference (P>0. 05),but in the observation group,their intraoperative hemorrhage, and length of hospital stay were significantly lower than those of the control group (P<0. 05);In the observation group,only 1 case had pulmonary infection,the incidence of complications was 17. 1%. In the control group,the incidence of complications was 40. 0%. The incidence of complications of the observation group was obvi-ously lower than that of the control group (P=0. 022). 3-month,6-month life quality of patients in the observation group was ob-viously higher than that of the control group (P<0. 05),but there had no significant difference between the 2 groups in 12-month life quality(P>0. 05);In the observation group,3-year survival rate of the observation group and the control group were 29. 3%, 30. 0% respectively,there had no significant difference between the 2 groups (P=0. 994). Conclusion Video-assisted thoraco-scope lung resection for non-small cell lung cancer have small trauma,less intraoperative bleeding,less postoperative complica-tions,and high postoperative survival quality. However,3-year survival rate of the 2 groups has no obvious differences,the recent clinical effect is satisfactory.
