肾盂癌的临床诊断和治疗(附32例报告)

目的探讨肾盂癌的诊治方法，提高肾盂癌的诊治水平。方法回顾性分析32例肾盂癌患者的临床资料。结果32例均经手术治疗，病理检查提示移行细胞癌29例，其中G1级8例，G2级12例，G3级9例，低分化鳞癌3例；Rubenstein分期：T1期11例，T2期9例，T3期6例，T4期6例。结论肾盂癌的诊断主要依靠临床表现和影像学检查，联合应用输尿管镜、尿脱落细胞学检查及尿NMP22水平的测定可提高正确诊断率。根治性肾、输尿管切除术是主要的治疗方法，应重视腹腔镜等在治疗中的作用。

Clinical Diagnosis and Treatment of Carcinoma of Renal Pelvis (Report of 32 Cases)

Objective To study the experience of diagnosis and treatment of carcinoma of renal pelvis.Methods The clinical data of 32 patients with carcinoma of renal pelvis were analyzed retrospectively.Results All the cases were treated surgically. The pathological type of 29 cases was transitional cell carcinoma (8 cases of G1, 12 cases of G2, 9 cases of G3), and of 3 cases was squamous cell carcinoma. Among them 11 cases were in T1, 9 cases in T2, 6 cases in stage T3, and 6 cases in T4.Conclusion The diagnosis of carcinoma of renal pelvis mainly relies on clinical manifestations and radiographic findings. The combined application of examinations such as ureteroscope, urine cytology, urinary nuclear matrix protein 22 may increase the rate of correct diagnosis of pyelic carcinoma. Total nephroureterectomy with removal of a cuff of periureteral bladder remains to be the choice of treatment for most cases.
