摘    要:
目的 :探讨经唇下 单侧鼻腔 蝶窦入路显微手术治疗鞍区肿瘤方法。方法 :CT扫描或MRI明确手术适应症38例 ,手术均采取经唇下 单侧鼻腔 蝶窦入路显微手术切除肿瘤 ,其中 5例采用扩大经蝶入路。结果 ;全切 2 3例 ,次全切除 11例 ,部分切除 4例。视力视野恢复 17例 ,并发症主要有尿

Abstract:
Objective: To evaluate the therapeutic efficacy of transsphenoid microsurgery resection of tumors in sellar region. Methods: 38 patients with tumors in sellar region diagnosed by CT or MRI scan were treated with transsphenoid microsurgery in this hospital between March 1996 and November 2000. Operations were performed via sublabio unilateral septo sphenoidal approach (5 cases were performed via extended transsphenoidal approach). Results: Among the 38 patients, gross total removal of the tumor were achieved in 23 cases, subtotal removal in 11 cases, and partial removal in remain 4 cases. Diminished visual activity and visual field defects were improved in 17 cases. The major complications after operation were diabetes insipidus and cerebrospinal fluid rhinorrhea, which occurred in 15.8% and 5.2%, respectively. No patient showed the complication of postoperative nasal tip deformities, septal perforations, or incisor tooth numbness. There was no death in the studied group. Conclusion: Microsurgical technique via transsphenoidal approach is a safe and effective one for the treatment of partial tumor of sellar region.
