高血压病患者动态血压变异性与微循环变化的关系

摘    要:目的 :探讨高血压病 (EH)患者的动态血压变异性 (ABPV)、甲襞微循环变化特点及二者的关系。方法 :以 3 0例年龄相当、血压正常的健康者作对照组观察了 67例EH患者的动态血压变异性(ABPV)、甲襞微循环变化。结果 :发现高血压病组微循环的管袢形态、血流流态、袢周状态积分值及总积分值明显高于正常对照组。相关分析结果显示 :正常对照组甲襞微循环总积分值仅与年龄呈弱相关 (r=0 .3 75 ,P <0 .0 5 ) ,与偶测血压 (CBP)、动态血压的各参数平均值 (ABP)、ABPV无显著性相关。高血压病组总积分值主要与CBP、ABP呈显著正相关 ,与 2 4h、夜间的ABPVs、ABPVd呈显著负相关 ,其中与 2 4hABPs关系最为显著 (r =0 .60 4,P <0 .0 1)。结论 :高血压病昼夜血压水平与血压的变异性 ,对微循环恶化改变的发生发展起重要的作用。

Abstract:Objective: To study the ambulatory blood pressure variability(ABPV)、nailfold microcirculation and relation between of them.Method: 67 patients of essential hypertension(PG) underwent ambulatory blood pressure monitoring and nailfold microcirculation,Thirty age matched normotensives were studied as control group(CG).Results: The shap、flow state,perivascular value and total integral value of nailfold microcirculation in the PG were higher than in the CG( P< 0.01 ).The results derived from correlation analysis showed the total integral value of nailfold microcirculation correlated bositively to the clinic blood pressure(CBP) and average levels of ambulatory blood pressure(ABP)and correlated negatively only to the 24 ABPVs,24 ABPVd,n ABPVs,n ABPVd in the PG.Thus the relation between the total interal value of nailfold microcirculation and ABPV was weaker than that between the total integral value and the average levels of ABP.The relation between the total integral value and the systolic average levels of 24 ABPs was the strongest( r= 0.604 ,P< 0.05 ).Conclusion: The changes of microcirculation was prominent in essential hypertension.The 24 h average levels of ABP and circadian changes of blood pressure may be an important role in the nailfold microcirculation disturbance.
