基层医院门诊老年患者用药依从性调查分析

目的了解基层医院老年患者用药依从性状况,寻求提高基层医院老年患者用药依从性的方法。方法采用问卷调查的方式,对本院门诊218名老年患者的用药知识来源、能否看懂药品说明书、用药理念、未依从用药的方式和原因、提高服药依从性的方法等进行调查。结果基层医院老年患者用药知识主要依靠医师的指导,其次是药品说明书;45%的患者能初步看懂药品说明书;用药理念是轻症尽量不用药,必须用药也要尽量少用药;未依从用药情形以忘记用药、医生态度和技术不佳、害怕副作用和症状缓解后误认为可不用药为主。结论医院药师应主动与医生沟通,共同做好用药依从性的宣传工作,门诊药房应设立药物咨询窗口,让更多患者了解如何看懂药品说明书、发生不良反应如何自救等问题。

Investigation and Analysis of Medication Adherence of Elderly Patients in Outpatient Department of the Grassroots Hospital

Objective To approach methods for improving medication adherence of elderly patients through comprehending the current situation of medication adherence of elderly patients in the outpatient department in the grassroots hospital.Methods By means of questionnaires,218elderly patients of the outpatient department in our hospital were investigated on their medication knowledge sources,whether they were able to read the package insert,medication concept,the pattern and reason for non-compliance, and measures for improving medication adherence.Results Medication knowledge of elderly patients mainly came from the instructions of physicians and secondly the package insert.45%of patients could primarily understand the package insert.Patients held the medication concept that medication was not to be taken as far as possible in event of mild diseases,and medication could be taken as less as possible in event that the medication was necessary.The main reasons for non-compliance were patients′forgetfulness,concern about side effects,false discontinuation of medication after symptom relief and physicians′ill manner or poor medical skills.Conclusion Pharmacists should take the initiative to communicate with the physician, work together to propaganda the importance of medication adherence.Outpatient pharmacy should be established with advisory windows,to help more patients to understand how to read the package insert and how to conduct self-rescue in event of adverse reactions.
