经Poppen入路显微手术切除松果体区肿瘤31例临床分析

背景与目的:松果体区病变位置深在,周围解剖结构复杂,直接手术切除病变是一个挑战。本研究旨在探讨松果体区肿瘤外科切除的Poppen入路及其技术要点。方法:对31例经Poppen入路切除的松果体区肿瘤进行回顾性分析,总结其临床特点、手术要点及预后。结果:肿瘤全切除26例,次全切除3例,部分切除2例,手术死亡2例;随访3个月至6年,KPS评分90分以上26例。结论:Poppen入路是治疗松果体区肿瘤较安全而有效的最佳入路之一,经脑生理间隙进入,肿瘤全切率高,手术创伤小,治疗效果好。

Microsurgical Management for Tumors in Pineal Region through the Poppen Approach:31 Case Experience

BACKGROUND & OBJECTIVE: A pineal region tumor is a deep-seated lesion of the brain surrounded by major vascular and neural structures. It is a surgical challenge to neurosurgeons. This was to report and review the use of Poppen approach surgical resetion of pineal region tumors. METHODS: Thirty-one cases with tumor in pineal region resected through Poppen approach were analyzed retrospectively. The clinical characteristics, operative technique and outcome were presented. RESULTS: Total removal was achieved in 26 cases. Three cases showed near-total removal and 2 cases showed partial removal. There were 2 mortality. The follow-up period ranged from 3 months to 6 years. Twenty six patients maintained Karnofsky Performance Scale (KPS) of 90 or more. CONCLUSIONS: Poppen approach is one of the safe and effective approaches for resection of tumors in pineal region. This is to approach the tumor through the natural anatomical corridor and it minimises tissue injury in surgery. We could achieve a high percentage of total removal and satisfactory outcomes.
