摘    要:目的探讨辅助生殖技术中影响玻璃化冻融胚胎移植结局的相关因素。方法对304例共323个周期的相关资料进行回顾性统计学分析,根据患者年龄、胚胎发育时期、移植日子宫内膜厚度、移植过程是否顺利和解冻后胚胎复苏程度进行分组,比较各类分组的胚胎种植率和临床妊娠率。结果受精第2天冷冻者与受精第3天冷冻者的胚胎种植率和临床妊娠率差异无统计学意义(P〉0.05);子宫内膜厚度和移植过程是否顺利组间胚胎种植率及临床妊娠率均无显著性差异(P〉0.05);不同年龄和冷冻胚胎复苏程度对胚胎种植率和临床妊娠率有显著影响(P〈0.05)。结论在冻融胚胎移植周期,患者年龄和解冻后胚胎质量对妊娠的成功率起主要作用,准备适宜的子宫内膜厚度能提高临床妊娠率。

Abstract:Objective: To explore the factors influencing the outcomes of the vitrified-thawed embryo transfer in assisted reproductive technology. Methods: A retrospective analysis was performed in 323 thawing cycles of 304 patients in our centre. The cycles were grouped according to the age of patients, development stage of frozen embryos, survival rate of embryonic cells after thawing, process of embryo transfer and endometrial thickness. Implantation rate and clinical pregnancy rate were compared between groups. Results: No significant difference was found in the implantation rate or pregnancy rate in the patients with different endometrial thickness, different development stages of embryos (frozen on day2 or day 3) transferred or different processes of embryo transfer. Compared with the patients younger than 35 years old, the patients aged over 35 years achieved lower implantation rate and clinical pregnancy rate (P〈 0.05). The embryos with incomplete survival of cells after thawing resulted in lower implantation rate and clinical pregnancy rate than those with complete survival of cells (P〈0.05). Conclusions: In vitrified-thawed embryo transfer cycles, the age of the patients and embryo quality play a major role in pregnancy rate. Preparation for appropriate endometrial thickness can improve the clinical pregnancy rate.
