TCD联合颈动脉超声对缺血性脑血管疾病的诊断效能

摘    要:目的:探讨经颅多普勒超声(TCD)联合颈动脉超声对缺血性脑血管疾病的诊断效能。方法:选择2017年10月~2019年11月某院神经内科收治的80例高度疑似缺血性脑血管疾病患者,均行TCD、颈动脉超声及数字减影血管造影(DSA)检查,以DSA检查为"金标准",分析TCD联合颈动脉超声诊断效能。结果:80例患者经DSA检测出血管狭窄68例,TCD联合颈动脉超声检查为63例,两种方法检测血管狭窄情况对比,差异无统计学意义(P>0.05);将DSA诊断作为"金标准",TCD联合颈动脉超声检查的准确度为91.25%、灵敏度为91.18%、特异度为91.67%、阳性预测值为98.41%、阴性预测值为64.71%。结论:TCD联合颈动脉超声诊断缺血性脑血管疾病具有较高的诊断效能,灵敏度、特异度均较高,为临床诊断、治疗缺血性脑血管疾病提供参考依据。

Abstract:Objective:To investigate the diagnostic efficacy of transcranial Doppler ultrasound(TCD) combined with carotid ultrasound in the diagnosis of ischemic cerebrovascular disease. Methods:80 patients with highly suspected ischemic cerebrovascular disease treated in the department of neurology of a hospital from October 2017 to November 2019 were selected. All patients underwent TCD, carotid ultrasound, and digital subtraction angiography(DSA). DSA was used as the "gold standard", to analyze the diagnostic efficacy of TCD combined with carotid ultrasound. Results: 68 patients had vascular stenosis detected by DSA, and 63 patients had TCD combined with carotid ultrasonography. There was no significant difference between the two methods in detecting vascular stenosis(P>0.05). DSA diagnosis was used as the "gold standard", the accuracy of TCD combined with carotid ultrasound was 91.25%, the sensitivity was 91.18%, the specificity was 91.67%, the positive predictive value was 98.41%, and the negative predictive value was 64.71%. Conclusion:TCD combined with carotid ultrasound in the diagnosis of ischemic cerebrovascular diseases has high diagnostic efficiency, high sensitivity and specificity, which provides a reference for clinical diagnosis and treatment of ischemic cerebrovascular diseases.
