研究PIVAS药师干预肠外营养不合理医嘱效果

目的:对PIVAS(静脉药物配置中心)药师对肠外营养不合理医嘱干预措施和效果进行分析。方法:选择某院PIVAS 2017年1月~2017年12月肠外营养医嘱共计5900组作为本文研究的实验组,另选择2018年1月~2018年12月肠外营养医嘱共计4700组作为本文研究的对照组。对照组该阶段不予以做任何干预,研究组该阶段予以综合干预,比较干预前后不合理医嘱的发生率变化。结果:较之于干预前的4.4%,干预后的临床不合理用药比例为0.8%,相比显著下降(P<0.05);PIVAS肠外营养不合理医嘱主要表现在液体容量比不当、pH值高、电解质稳定性、超说明书剂量用药、重复用药及其它。结论:药师对PIVAS肠外营养不合理医嘱进行有效干预,能有效减少不合理用药情况的发生,保证静脉用药安全性。

Study on the Effect of PIVAS Pharmacists' Intervention on Unreasonable Medical Advice of Parenteral Nutrition

Objective:To study the measure of PIVAS(Pharmacy Intravenous Admixture Service)pharmacists'intervention on unreasonable medical advice of parenteral nutrition and to analyze the effect.Methods:A total of 5900 groups of parenteral nutrition medical advice in PIVAS of a hospital from January 2017 to December 2017 were selected as the experimental group,and 4700 groups of parenteral nutrition medical advice in PIVAS of a hospital from January 2018 to December 2018 were selected as the control group.The patients in the control group were not given any intervention at this stage,and the patients in the study group were given comprehensive intervention at this stage,and the incidence of unreasonable doctor's orders before and after intervention was compared.Results:Compared with 4.4%before intervention,the proportion of unreasonable clinical use after intervention was 0.8%,which was significantly decreased(P<0.05).PIVAS parenteral nutrition unreasonable medical advice is mainly manifested in improper liquid volume ratio,high pH value,electrolyte stability,off-label dose medication,repeated medication and other.Conclusion:PIVAS pharmacists'intervention on unreasonable medical advice of Parenteral Nutrition can effectively reduce the occurrence of irrational drug use and ensure the safety of intravenous medication.
