双J管留置时间与并发症分析

目的探讨双J管留置时间与并发症的关系及处理。方法将325例患者随机分成Ⅰ(保留双J管时间1个月)、Ⅱ(保留双J管时间2个月)组,回顾性分析应用双J管的时间与并发症。结果两组均出现有不同程度的并发症,Ⅰ、Ⅱ组分别为:膀胱刺激症40%、85%;肉眼血尿60%、75%;下腹部不适及腰痛32%、83%;双J管上移5%、8%;尿液反流7%、8%;严重尿路感染伴发热1%、1.4%;取管困难0.8%、6%。结论双J管的应用对防止泌尿系手术后漏尿和狭窄、术后恢复具有重要的作用,但也存在应用并发症,与安放时间存在一定关系,较轻者有膀胱刺激症状、血尿、腰痛、管壁尿盐沉着,严重者有双J管移位、较重的尿路感染、取管困难等。在应用过程中要注意安放时间的选择及对并发症的处理。

Analysis of Complications and Time Indwelled about Double-J

Objective To evaluate the relationship and management of complications and time indwelled about Double-J after ureteroscopic lithotripsy. Methods 325 cases of ureter stone were assigned into two groups randomly, group I （indwelling 1 month） and group H （indwelling 2 months）.A retrospective study was performed about duration of indwelling and complications. Results The two groups occurred complications more or less, Ⅰ and Ⅱ respectively： bladder irritalive symptoms 40%, 85%; hematuria 60%, 75%; hypogastrium discomfort and lumbago 32%, 83%; Double-J moving 5%, 8%; backflow of urine 7%, 8%; serious urinary tract infection with fever 1%, 4%; dysextraction of Double-J 0.8%, 6%. Conclusion It is important to prevent leakage of urine and ureterostegnosis after postoperation on the application of Double-J. But complication is common, and time-related. Lighter complications were bladder irritative symptoms, hematuria, lumbago, salt-urine deposit, severe others were Double-J moving, serious urinary tract infection, dysextraction of Double-J and so on. The indwelling time selection and management of complications is extremely important at the application of this procedure.
