羟喜树碱联合白介素-2灌注预防非肌层浸润性膀胱癌术后复发的疗效观察

目的 评价羟喜树碱(HCPT)联合白介素-2(IL-2)膀胱灌注预防非肌层性膀胱癌(NMIBC)术后复发的疗效和安全性.方法 2011年9月至2013年9月本院收治的浅表性膀胱癌152例,随机分为对照组(HCPT)和联合组(HCPT+ IL-2),两组患者行经尿道膀胱肿瘤切除术(TURBT)后即刻开始行灌注治疗,每周1次,连续8周后改为每月1次,治疗总周期1年,比较两组患者术后复发率及不良反应.结果 完成2年随访后,对照组复发18例,复发率为23.6％(18/76),联合组复发8例,复发率为10.5％ (8/76),差异有统计学意义(x2=4.640,P＜0.05),膀胱刺激征及血尿等差异无统计学意义(x2=1.689,P＞0.05).结论 HCPT联合IL-2灌注预防NMIBC的效果优于单用HCPT,不良反应率低,耐受性好,可作为灌注的一线用药.

Efficacy observation of hydroxycamptothecin combined irrigation of interleukin-2 against postoperative recurrence of non-muscle invasive bladder cancer

Objectives To evaluate the efficacy and safety of hydroxycamptothecin(HCPT) combined irrigation of interleukin-2 against postoperative recurrence of non-muscle invasive bladder cancer (NMIBC).Methods 152 patients with NMIBC were radomly divided into control group (HCPT) and combination group (HCPT + IL-2).After trans-urethral resection bladder tumor (TURBT),all patients received perfusion therapy,once a week for eight weeks to once a month,a total of one year.The recurrence rate and adverse reaction were observed in two groups.Results Follow up 2 years,recurrence rate in control group and combination group was 23.6％ (18/76),10.5％ (8/76) respectively.There was statistically significant difference (x2 =4.640,P ＜ 0.05),and there was statistically significant difference in adverse reactions (bladder irritation and blood in the urine) between the two groups (x2 =1.689,P ＞ 0.05).Conclusions HCPT combined irrigation interleukin-2 against postoperative recurrence of NMIBC is better than hydroxycamptothecin only,which has the advantages of low rate of adverse reactions,good tolerance as a first-line therapy of drug perfusion.
