F6/7.5输尿管镜联合钬激光碎石治疗小儿输尿管结石

目的 探讨F6/7.5输尿管镜联合钬激光碎石治疗小儿输尿管结石的疗效.方法 回顾性分析2010年6月至2015年9月收治的29例采用F6/7.5输尿管镜联合钬激光碎石治疗的小儿输尿管结石的临床资料.结果 28例行输尿管镜联合钬激光碎石成功,l例因进镜失败改开放手术取石.手术时间30～80min,平均46min,2例输尿管上段结石术中部分较大结石碎片冲入肾盂,术后行体外碎石排出.术后1例出现发热,予抗感染治疗后体温下降.术后血尿2～4d消失,4～7d出院.术后4周复查CT未见明显结石残留,肾积水恢复正常或已明显改善.结论 F6/7.5输尿管镜联合钬激光治疗小儿输尿管结石,成功率高,并发症少,疗效满意,是治疗小儿输尿管结石的良好方法.

Treatment of ureteral calculi in children with F6/7.5 ureteroscopic holmium laser lithotripsy

Objectives To evaluate the efficacy of F6/7.5 ureteroscopic Holmium laser lithotripsy in the treatment of childern with ureteral calculi.Methods Clinical data of 29 children with ureteral calculi who underwent F6/7.5 ureteroscopic lithotripsy combined with holmium laser from June 2010 to September 2015 were retrospectively analyzed.Results The lithotripsy was successful in 28 these patients,1 case was cured by ureterolithotomy after failed ureteroscope therapy.The operation time was 30 ～ 80min,mean 46 min.Two patients of proximal ureteral calculi because of the larger pieces of stone into the renal pelvis in operation were followed ESWL.Postoperative fever occurred in 1 case,cured by conservative treatment.Gross hematuria was disappeared in 2 ～ 4d,discharge time was 4 ～ 7d.4 weeks after operation,the CT examination was performed,there were no obvious residual calculi in all patients.Hydronephrosis was dispeared or improved significantly.Conclusions F6/7.5 ureteroscopic holmium laser lithotripsy is effective to manage ureteral calculi in children,with advantages of high success rate,few complications and satisfactory curative effect.
