一次性包皮环切缝合器与商环在包皮环切术中的临床应用比较

摘    要:目的 比较一次性包皮环切缝合器(DCSD)与商环在治疗包皮/包茎中的安全性和疗效比较.方法 选取本院就诊的包皮过长/包茎患者180例,运用随机数字表法分为DCSD组(n=93)和商环组(n=87),比较两者治疗包皮过长/包茎安全性和疗效等方面指标.结果 DCSD组和商环组手术时间分别是(6.36±1.09)和(7.23±1.10) min,术后24h疼痛评分分别是(1.94±1.08)和(2.89±0.98)分,术后7d疼痛评分分别是(2.08±0.89)和(3.74±1.01)分,愈合时间分别是(18.44±3.58)和(21.66 ±3.65)d,术后切开裂开发生率分别是2.2%和11.5%,术后满意率分别是86.0%和73.6%,上述指标两组具有统计学差异(P<0.05);两者在术中失血量、术后水肿及感染发生率方面没有差异(P>0.05).结论 DCSD包皮环切术具有手术时间短、术后疼痛轻、愈合时间短以及外观满意率高等特点,值得临床推广.

Clinical application of disposable circumcision suture device (DCSD) versus ShangRing circumcision in male circumcision
Abstract:Objectives To compare the efficacy and safety of disposable circumcision suture device (DCSD) and ShangRing circumcision in the tratment of patients with redundant prepuce or phimosis.Methods 180 patients with redundant prepuce or phimosis were randomly divided into two groups:the DCSD group (n =93) and the ShangRing group (n =87).Factors concerning efficacy and safety were compared.Results DCSD showed obvious advantages over the ShangRing in operating time (6.36 ± 1.09) min vs.(7.23 ± 1.10) min],postoperative 24h pain score (1.94 ± 1.08) vs.(2.89 ± 0.98),postoperative 7d pain score (2.08 ± 0.89) vs.(3.74 ± 1.01),wound healing time (18.44 ± 3.58) d vs.(21.66 ± 3.65) d],the incidence of wound dehiscence (2.2% vs.11.5%),and overall satisfactory rate (86.0% vs.73.6%).There was significant difference in above results (P < 0.05).There was no statistically significant difference in intraoperative blooding loss,edema,and wound infection between the two groups(P > 0.05).Conclusions DCSD has advantages of shorter operation and incision healing time,reduced postoperative pain score,and high appearance satisfaction.It should be widely used in clinical practice.
